Objective: Rheumatoid arthritis(RA) is an autoimmune disease characterized by chronic synovial inflammation and destruction of joints. The etiology and pathogenesis of RA remain unknown. The prominent pathologies of RA are observed: synovial infiltration of multiple inflammatory cells mainly composing of T lymphocyte, synovial lining cells proliferation, angiogenesis and pannus formation. Local active proliferative lymphocytes and superproliferative synoviocytes play a key role in synovial inflammation which leading to progressive damage of articular cartilage and bone of patients with RA. Cytokines are important regulators in synovial inflammatory reaction and immune response, which posses different functions such as pro- or anti-inflammation and positive or negative feedback of immunoloregulation, affecting in the process of synovial pathological changes.RA belongs to the category of "arthralgia syndrome" in traditional Chinese medicine. Wind-cold-dampness evil invading into the body, phleym-wetness stagnating the local meridians and then leading to swelling and malformation of joints are its main pathogenicities. Gancaofuzi-tang is a classic prescription on treating wind-cold-dampness type of arthralgia. Tongbi-fang based on it and adding herbs having property of promoting blood circulation to remove obstruction in the meridians, which has gain better curative effect on RA treatment in clinical application. By CIA rats model, we observed the effect of Gancaofuzi-tang and Tongbi-fang oninfluencing synovial T-cell subsets and adjusting the expression ofcorrelative cytokines, further ameliorating the phenotype of jointhistopathology. Our studies investigate the mechanismsanti-inflammation and immunoloregulation of Chinese medicinewhich have the roles of warming the channel to promoting the flow ofqi and activating blood circulation to remove blood stasis on RAtreatment.Method:1 .Type II collagen induced arthritis model in rats were established.2.The feet swelling degrees of rats were measured, then calculateswelling rate. 3.The level of serum SA was detected and the level of serum IL-2 andTNF- a were determined by ELISA. 4.1mmunohistochermstry techniques have been used to detect T-cellsubsets, cytokines TGF- ?and FGF in synovial tissue of CIA rats.The above results were analyzed with image analysis system. 5.The synovial histopathology and ultrastructural of rats wereobserved with optical microscope and electronic microscoperespectively. Results: 1 .CIA rats have clinical situation with obviously persistent swelling oflocal joints with the less red and hot. The manifestations of synovialhistopathology are lymphocytes infiltration, lining cell proliferation,angiogenesis and pannus formation. 2.Gancaofuzi-tang and Tongbi-fang significantly decreased swellingrate of joints of CIA rats. The pathology of synovisl tissues weremarkedly ameliorated observed with optical and electricmicroscopies. 3.Gancaofuzi-tang and Tongbi-fang can markedly reduce the level ofserum SA of CIA rats compared with control on 3 week. After continuous administration for 6weeks, the serum SA of GF and TB groups can drop to approach the normal level.4.After administration for 3 weeks, the levels of serum IL-2 and TNF-a of rats in GF and TB groups were significantly decreased compared with control. Continuous administration till 6 weeks, the levels of GF and TB rats achieved to near the normal level. 5.Gancaofuzi-tang and Tongbi-fang effectively reduced CD4+ T-cell expression on synovium of rats, meanwhile, Gancaofuzi-tang also increased the expression of CD8+ T-cell, resulting in the lower ratio of T-cell subsets. 6.Gancaofuzi-tang and Tongbi-fang up-regulated TGF- ? anddown-regulated FGF cytokines expression in synovium of rats.
