Tumor which ranks second cause of human mortality keeps increasing gradually in the past decades, and becomes a main threat to human health. Gastric cancer in particular is the most common and dangerous malignant cancer in digestive tract. It is putative thought that tumor invasion is an important process in the progression of various cancers cell and most malignant tumor patients died for the infiltration and metastasis of tumor though the molecular mechanism of tumor invasion keeps elusive to date. A series of complicated interaction of tumor cell, host cell and extracellular matrix through more steps result in tumor invasion and metastasis. The destruction of basement membrane integrity is a start signal of malignant tumor invasion. Matrix metalloproteinases (MMPs) play an important role in this degradation of the extracellular matrix. The major component of basement membrane is type IV collagen and degradation of this collagen is preferentially performed by specific collagenase/gelatinase, such as the MMP-2, 9 forms. Further studies implied that the activated MMP2, 9 other than Pro-MMP2, 9 have essential roles in invasion and metastasis of gastric cancer, However, little information in domestic is available with regard to their roles in tissue and peripheral blood from patients with gastric carcinoma. In this study, we examine the total/activated MMP2, 9 expression in tissues and serum from patients with gastric carcinoma, so that the correlation between activated MMP2. 9 expression and tumor invasion could be evaluated.Materials and MethodsTissue specimens: The subjects in this study were patients with gastric carcinoma who were diagnosed by pathology after surgical resection and taken from First affiliated hospital of zhejiang university between 2000 and2 ?籊i. There v/er^ -<2 r^alej and il ferrules, '.vith an age range of 56 76 years (median age, 63.2 years). Of those 23 cases are adenocarcinoma, 4 cases are undifferentiated carcinoma, 8 cases are mucoid carcinoma, 8 cases are signet-ring carcinoma; 35 cases are with node invasion while 8 cases are without node invasion. The gastric mucosa which were more than 5cm from adjacent cancer is thought to be normal.Serum specimens: It includes 43 gastric cancer patients and 48 non-gastric cancer patients. Controls included 13 cases with chronic superficial gastritis, 19 cases with chronic atrophy gastritis and 16 cases with chronic superficial- atrophy gastritis. There were 29 males and 19 females, with an age range of 40-70 years (median age, 51.4 years). All above subjects were verified by endoscopy and pathology. Quantitative determine of the total/activated MMP2, 9 in specimen by ELISA.ResultsThe total /activated MMP-2, 9 of tissue and peripheral blood in gastric cancer patients is much higher than that of non-gastric cancer patients obviously. There are statistical differences between two groups (p<0.01 and p<0.05). The activated MMP-2 in patients with node invasion is much higher than that of patients without lymph node invasion (p<0.05). However, there is not relationship between lymph node invasion and expression of total/ activated MMP-2. 9 in peripheral blood, total MMP-2, 9 and activated MMP-9 in cancer tissue.Conclusions1. MMP-2, 9 can be secreted by both tumor cell and stroma ceil which was induced by tumor cell.2. MMP-2, 9 in peripheral blood may be a better referenced indicator for the diagnosis of gastric cancer and a theoretic foundation of therapy with synthetical TIMPs.3. The overexpression of activated MMP-2 in tissue is correlated with invasion of lymph node, the detection of activated MMP-2 in tissue can be an indicator to evaluate the prognosis of gastric cancer patients after surgical operation.4. ELISA for detection of activated MMP-2, 9 is non-invasive, simple and it is worthy of being extended to clinical diagnosis.
