Labiate intestinal fistula can hardly be cured with non-operation method in clinic.Generally.it should be packed fistula orifice with silastic film to control intestinal juice and recover enteral nutrition, than it was cutted 3-6 mouths later.Pedicled muscle flap has been widely used in experiment and clinic.Whether pedicled straight muscle of abdomen flap could be abopted to pack fistula orifice and to cure the Labiate intestinal fistula has not been reported up to now at home and abroad. To investigate the possibility of operation which packing the fistula orifuce with pedicled straight muscle of abdomen flap to cure the labiate intestinal fistula by experimental study and to investigate its healing mechanism by autopsy and pathologic examination. The study will provide the experimental foundation to cure the labiate intestinal fistula .Material and Metheds1. Rabbits:20 healthy rabbits were randomly assigned to one of two groups.Both control and experimental animals were underwent4ileostomy and were made Labiate intestinal fistulas model. 10 rabbits were packed the fistula orifices with pedicled straight muscle of abdomen flap .another 10 rabbits' ileostomy as control group.2. Labiate intestinal fistula model:Each of the animals was made ileostomy 20cm from the cecum. .the diametre of fistula orifice was about 1.0 cm.3. The operation methed of experimental group:After being succeed in ilostomy and being calculated precisely.the middle-inferior of straight muscle of abdomen was incised about 1.0cm transversely and 4-5cm upward vertically. In order to pack the fistula orifice complctely.the tip of muscle was easily drew to fistula orifice and was sutured to the intestinal wall4. Specimen fabrication and histological examination: Five of the ten in the experimental group were put to death respectively the 15th and the 3()th postoperative day.. We took the intestinal tract including the fistula and the muscle flap, then observed the color and texture of muscle flap and the healing condition of the fistula and the muscle flap.The seven stocked rabbits of control group were put to death on the tenth day after operation. We aslo obeserved the healing condition of the fistulas and the wound.Results1. General conditions:All the animals were alive after orperation.10 rabbits of control group got infected in incision Their fistulas" diameter were enlarged slightly.Three rabbits of control group were died5respectively on the 5th .the 7th .the 8th postoperative day. 24-hours drainage of control group averaged 95ml(range:70-l80ml).The 10 rabbits of experimental group were all alive and their fistulas were all healed.2. Observed in naked eyes.In the 10 rabbits of the experimental group .the healing conditions of the fistulas and the muscle flaps were satisfactory.The surface of the muscle flap was covered with granulation tissue on the 15th postoperation day. yet the edema of the subcutaneous tissue was obvious.And on the 3()th postoperation day the muscle flap was covered with the granulation tissue compeletely .The fistulas of 10 rabbits of control group enlarged rather than being healed..The intestinal tracts of the dead rabbits were not obstructed, and general infection is the most possible cause of death .3 Histological examination(seen by the optical microscope) :The striated muscle cells of the heel of packed straight muscle of abdomen flap were basically normal(Figurel) ..The striated muscle cells of the tip of packed straight muscle of abdomen flap ended in degeneration and necrosis. A large amount of fibroblasts and blood capillary hyperplasia form granulation tissue .which result in the healing of fistulas and muscle flap(figurc2,3.4).The surface of granulation tissue was not crawled intestinal mucosa epithelial cells on the 15th postoperation day(Figure5),but was crawled intestinal mucosa epithelial cells on the 30th postoperation day(Figure6).ConclusionOn the condition that the intestinal tract is unobstru...
