Objective To investigate the value of echocardiography and magnetic resonance imaging(MRI) in earlier diagnosis of aortic dissection.Methods TTE: The patient lies in the left position. First, routine detect and then main detect the section of aortic by Sonos 5500 Color Doppler Ultrasonic Detector using frequency of transducer 2-4MHz. To observe aorta and its branch vessel dimension, thickness of the wall, intimal flap, false lumen ,thrombus, two lumens blood flow rate , entry and exit of the dissection, the presence of aortic regurgitation, pericardial effusion and so on. MRI:The patient lies on his back. Using TORSO coil, SE array,cross section and bristle section, T,WI and T2WI, MRI or MRI+MRA. To observe the beginning site of aortic false lumen, intimal flap, dimension of true and false lumen, thrombus in false lumen. We can see the blood in false lumen coming from true lumen. The main observation is the entry site of thoracic aortic dissection,true and false lumen of aortic branches, thickness of aortic wall and so on.Results 25 surgical patients were examined by Doppler echocardiography and Magnetic resonance imaging. The ratio of right diagnosis is 96%, 100%; dividing into groups is 92%, 88%; detecting rupture is 76%, 88%; detecting thrombus is 60%, 90% respectively; Consistent between intimal flap and operation is 72%, 50% respectively. Comparing operation with TTE and MRI, the ratio of right diagnosis is 100%, dividing into groups is 100%; detecting rupture is 92% and detecting thrombus is 92% respectively; Consistent between intimal flap and vivisection is 80%.TTE: 19 cases(76%) with aortic valvular involvement, 9 cases(36%) with pericardial effusion, 1case with cardiac tamponade. MRI: 2 cases with aortic branches involvement.Conclusion TTE is the first method in diagnosis of aortic dissection and add to MRI in thoracic aortic dissection and the bad quality of echocardiography.
