Objective To observe the long-term efficacy and safety of carvedilol in patients with idiopathic dilated cardiomyopathy(IDCM). Methods Thirty patients with IDCM were randomly divided into two groups. Fifteen patients who received conventional therapy ,including digoxin ,diuretics andangiotensin-converting-enzyme(ACE) inhibitors acted as control group. Fifteen patients were treated with carvedilol on the basic therapy of the control group . Patients administrated carvedilol 2.5mg twice daily for Iweek ,5mg twice daily for 2 weeks,lOmg twice daily for 2 weeks and 20mg twice daily to the target dose or to the tolerance. After 3 months, clinical ,cardiothoracic ratio and erhocardiograthy parameters were evaluated.Results carvedilol increased left ventricular ejection fraction (LVEF) from (33.4 ?. 5) %to (47.9 ?8. 2)%,and decreased left ventricular end-diastolic diameter(LVEDD) from (71.1 ?. 6)mm to(67.2?0)mm, left ventricular end-systolic diameter(LVESD) from (59.8?.8)mm to (49.9 + 6. l)mm, heart rate (HR) from (97.4 + 12.3 )bpm to (77. 8 + 12. 3)bpm, diastolic blood pressure (DBF) from (80. 3 + 7. 5)ramHg to (73. 3 + 10. DmmHg and cardiothoracic ratio from (0. 65 + 0. 04) to (0. 61 + 0. 06) .In addition , carvedilol improved NYHA class from (3. 2 + 0. 8) to (1.8 + 0.7). Conclusion carvedilol were well tolerated and safe in patients with IDCM for 3 months and diminished heart size and improved left ventricular systolic function and clinical symptoms...
