Hyaluronic acid (HA) is the main composition of extracellular matrix (ECM), witch takes play an important function in the earlier period of liver fibrosis and cell propagate, tumor cell transfer. In the procession of liver transplantation, the function of hepatocytes and liver sinusoidal endothelial cells suffered to the influence and serum hyaluronic acid level increased after liver transplantation because of cold ischemia-reperfusion. So the examination of serum hyaluronic acid level may be an available method of understanding and evaluating hepatocytes, liver sinusoidal endothelial cells function in the procession of liver transplantation. The purpose of this research is to observe the changes of circulation serum hyaluronic acid level at different periods and to investigate the meaning in the experimental piggyback liver transplantation operation.Materials and MethodsFifteen pairs of health large white pig were subjected to piggyback liver transplantation. Donor pigs were slightly smaller than recipient pigs. Experiment started with the muscle injection of Atropine 0.02mg/kg and Ketamine 8mg/kg. After anesthesia, the endotracheal tube was connected to the jet ventilation machine to provide oxygen. The donor liver was sliced and pruned. Then the donor liver was provided Albumin and cold liquid to maintain liver with lower temperature.The weight of recipient pigs averaged 28.3 + 5kg. After muscle injection of Atropine and Ketamine, Propofol 2mg/kg, Fentanel 0.002mg/kg, vecuronium O.lmg/kg were used to begin anesthesia induction. Along the right trachea side the skin was cut open to expose the jugular vein and carotid artery. The tube was placed into jugular vein to monitor the center vein pressure and into carotid artery to continuously monitor the artery blood pressure. During the surgical operation the medicine of Propofol, Fentanel and vecuronium were given by the vein to maintain anesthesia stability. Compound sodium chloride solution, Gelofusine, frozen blood plasma and pig fresh blood were used to maintain circulation stability. The operation method was adopted with non vein-vein bypass piggyback liver transplantation. During the animal experiment blood samples were taken respectively at the four different points of pre-operation phase, pre-anhepatic phase, anhepatic phase and neohepatic phase. The hyaluronic acid level was examined by emanation immunity analysis. In the same times the ALT, AST and y -GT of liver function were assessed. The data were handled by statistics and compared with three liver function indexes.ResultsThe average time of liver transplantation was 343.33?3.99mmutes, among that the average time from anesthetizing to the pre-operation phase was 82.34?1.56minutes and 146.13?1.61 minutes from pre-operation phase to pre-anhepatic phase. The average time of anhepatic phase was 47.07?.35minutes. Systolic pressure reduced about 23. 51 ?. 87minHg and diastolic pressure about 11. 35 ?. SlmmHg. The hyaluronic acid level gradually rose after anaesthesia, and it reached as high as 267 Dg/L before pre-operation. At the pre-anhepatic phase hyaluronic acid rose to the level of 1743 Dg/L (P<0.05 ). During the anhepatic phase serum hyaluronic acid quickly increased from 1743 Dg/L to 9530 Dg/L, obviously higher than other phases ( P<0.01 ) . When the donor liver transplanted and reperfused,the serum hyaluronic acid level descend to 1485 u g/L ,which was lower than pre-anhepatic phase and anhepatic phase. With comparing to the pre-operation phase the HA still maintained higher level (P<0.05) . From pre-operation phase to anhepatic phase, the ALT did not show a worthy change. After donor liver transplanted and reperfused, the ALT had a slightly increase (PO.05 ) . The AST continuously increased in the liver transplantation. It reached higher after donor liver transplanted (P<0.01) . Y -GT showed obvious increase in the anhepatic phase and quickly descended after reperfusion (P<0.05).
