Growth hormone(GH) is a type of monopeptide strain hormone ,which consists of 151 amino acid. It is released from anterior pituitary eosinophilic cells. GH takes effect on target tissue by combining with growth hormone receptor (GHR),as well as mediated by insulin-like growth factor(IGF).GH has been shown to increase gatric mucuous growth and promote adaption. So it is considered as an important nutrinent factor. Chronic atrophic gastritis (CAG) is well-known gastric precancerous lesion and list to first rank of cancer prevention by WTO. So CAG reversal therapy is urgent. Studying serum GH level on CAG patients is also important because GH takes role in mucuous nutrinent.ObjectivesTo investigate the difference level of serum GH between CAG group and control group and the relationship between level of GH and the severity of gastric atrophy and intestinal metaplasia in CAG petients.Patients and methods Patients31 patients (female 23 ,male 8;age 51.74?0.25) were enrolled in CAG group ,which was conformed by gastroendoscopical and pathological findings. We excluded the patients who had other vital organs (heart,liver,lung ,pancreas and pituitary et al ) abnormal findings. We recruited 31 health volunteers as control group (female 11,male 20;age 49.68?5.43). Both groups divided into three subgroups according to age as follows: 10 patients of young age subgroup (less than 45 years old);10 patients of median age subgroup(45~59 years old);10 patients of old age subgroup (more than 60 years old).Methods1. blood samples: 3ml blood were taken from elbow vein in morning. The level of serum GH were measured by fluorescence radioimmunoassay technique.2. tissue samples: biopsy are performed under gastroendoscopy .Tissue samples were sent to pathology and made into paraffin section. Hematoxylin eosin stain was used. Intestinal metaplasia were detected by histoimmunochernistry methods including HID, AB and PAS stain.3. statistical analysis: concentration of serum GH were expressed as percentile (pr) because data were non-standard distribution. All data were stored in SPSS 10.0 database and analysed with non- parametrie statistics.ResultsLevel of serum GH in CAG group was p25 0.05ng/ml, p50 0.07ng/ml and p75 0.16mg/ml respectively, while in control group was 0.17ng/ml,0.37ng/ml and 0.715ng/ml individually. The difference between two groups is significant. Average level of GH in young and median age subgroups in CAG patients was 0.06ng/ml and 0.07ng/ml respectively, compared with control subgroup 0.255ng/ml and 0.460 mg/ml was significant (p=0.02 and p=0.01 respectively). Compared with control old age subgroup 0.360ng/ml, the level 0.095ng/ml had a striking decrease in CAG subgroup, but no significant (p=0.052). We also revealed that the level of GH had an obvious decrease with the severity of atrophic change and intestinal metaplasia increasing (p=0.55 and p=0.078 respectively). In CAG group GH concentration is 0.09ng/ml in slight atrophic change, 0.115ng/ml in medium change and 0.05ng/ml in severe atrophy. From non to severe metaplasia lesion, GH level is 0.Ing/ml , 0.08ng/ml ,0.065mg/ml and 0.55mg/ml respectively .We can see GH level was decreased gradually with the degree of gastric atrophy and intestinal metaplasia increasing, which revealed that the inseperable correlation between GH level and the severity of CAG.DiscussionCAG is well known gastric precancerous status. It has been shown that there is high CAG morbidity in high tendency gastric cancer area and severity of atrophy , intestinal metaplasia and atypical hyperplasia around the cancer center focal. It was also found gastric cancer is susceptible to exist in gastric intestinal metaplasia area. So CAG has a close correlatioan with gastric carcinoma. It was suggested that CAG had a correlation with gastrc blood flow supplying and local nutrinent hormonelevel, particularly the gastrin level. In normal condition the level of GH spontaneously decreased with age in old people (age more than 50 years old)...
