Gastric cancer is a worldwide malignant tumor. Many people die from the invasiveness and metastasis of it . It is difficult to diagnose the metastasis preoperatively. Adhesion molecule CD44, the abundant cell surface transmembrane glycoprotein with multiple alternative splicing variant, has been implicated in a variety of biological processes including lymphopoiesis , angiogenesis , wound healing ,T and B lymphocyte activation and tumor metastasis . The precise role in these events is still unclearly.To assess the relationships between the expression of CD44 splicing viarant 5 (CD44v5) and the biological behaviors of gastric cancer , including tumorigenesis , development, differentiation , growth pattern , the depth of invasion and lymph node metastasis, as well as its diagnostic value . 102 formalin-fixed , paraffin-embodied samples , including 48 samples of gastric cancer , 8 samples of gastric mucosa near benign gastric ulcer , 13 samples of chronic superficial gastritis , 33 samples of dysplastic gastric mucosa , were selected to immunohistochemical staining with specific mouse anti-CD44v5 monoclonal antibody by streptavidin-peroxidase method (abbreviation: S-P).The result shows that CD44v5 was only expressed focally in the areas of intestinal metaplastic gastric mucosa in both 8 samples of gastric mucosa near the benign gastric ulcer and 13 samples of chronic superficial gastritis .The expression of CD44v5 was observed in 60.4% of carcinoma and 30.3% of dysplastic gastric mucosa withsignificant difference statistically(x2=7.103, P<0.05) . 76,9% of lymph node metastasis carcinoma was positively stained by CD44v5 , whereas 40.9% of non-lymph node metastasis carcinoma was stained by it (x 2=6.463 , P< 0.05). CD44v5 positivity was detected more frequently in well and moderately differentiated carcinoma than in poor differentiated and undiffrentiated carcinoma (73.3% versus 38.9% , x2=5.581, P<0.05). In different growth pattern, there is also a significant difference between the inflating-type and infiltrating-type gastric cancer (35.3% versus 78.9% , x2=7.256, P<0.05). No significant association was'found between the expression of CD44v5 and sexual, the degree of dysplasia,the depth of invasion in gastric cancer.We conclude that the overexpression of CD44v5 maybe involved in the tumorigenesis , lymph node metastasis , growth pattern and differetiation in gastric cancer . But it can not be regard as an independent diagnostic marker of the metastasis of gastric cancer. |
