AbstractThe Research of Propofol Brain Uptake duringtheTarget Controlled Infusion of PropofolObjective:To compare the correlationship between the arterial concentration(Ca) the jugular bulb venous concentration(Cjv) , the effective compartment concentration(ECC) and the calculated area under curve of the arterial- jugular bulb venous concentrations(AUC(ajv~)[mean the brain uptake] in different time during the target controlled infusion(TCI) of propofol.To discuss whether ECC can manifest the brain uptake of propofol.To proved whether the ECC inferred by mathematic is rational in the clinical pratice during the TCI of propofol.Method:Twelve ASA I II patients with neither cardiac nor intracranial disease,were scheduled for elective abdominal operations.After the epidural anesthesia ,adopting effect compartment modeling,computer controlled infusion propofol was performed in all patients.The target concentration of propofol was 4.00 I.?g.mr?The Ca and Cjv of propofol were measured simultaneous from the radial artery and the jugular venous bulb blood sample 1 ml by high performance liquid chromatography(HPLC) in different time.The AUC(a.jv) was calculated in the same time.The ECC of propofol was also recorded.Result:When ECC rose from 0 to 4.0 ii g.mF?4.7 ?.16mm ), AUC(a1v)was 11.47 1-?g.min?mV? AUC(a~jv) was correlated with FCC positively4(rECCAUC=O.977,P~O.OOI). Ca and Cjv were not correlated with AUC(a.jv) (rca~ AUC ~O.O54,P=O.92 ; rc~YAucO.335,P=O.5l6).When the infusion wascontionous and kept ECC4.OO ~t g.mU? Ca was not different with Cjv(P=~O.5 12).From stopping infusion to the patients?openning eye, AUC(a~jv) was -16.71 ii g.min?.mf?and also correlated with ECC positively (r~O.942,PO.OO5). Ca and Cjv were correlated with AUC(a.~~v) positively(r CaAUC 0.986,P0.00 rCIVAUC~O.974, P0.OO 1) too.Conclusion:During the effect compartment modeling computer-infusion of propofol, FCC is significantly correlated with AUC~a,v) . When the propofol is continous infused,the brain uptake of propofol keep equilibrium state.During the TCI of propofol,EEC can manfiest the brain uptake of propofol in different time,so the inferred ECC is rational during TCI of propofol.
