Purpose:Through compare clinical efficacy treadment of Lumbar instability of simple Cage posterior interbody fusion combined with pedicle screw fixation and in the front of intervertebral space Cage suppress fusion with bone graft combined with pedicle screw fixation, analyzes two planting bone way in interbody fusion of the effectiveness of clinical efficacy, stability, etc., to provide reference for clinical practice.Methods:By observing the clinical data of Lumbar instability of 60 patients that by posterior Cage bone graft fusion combined with Pedicle screw internal fixation,at the orthopaedic of affiliated hospital of Chengdu University of Traditional Chinese Medicine and the orthopedic surgery of Chengdu Second People’s Hospital,During the period from 2013 March to 2014 March.The follow-up time was 12 months.30 patients in the A group using a simple Cage posterior interbody fusion combined with pedicle screw fixation.30 patients in the B group,by in the front of intervertebral space Cage suppress fusion with bone graft combined with pedicle screw fixation.Two groups were preoperatively to postoperatively 1,3,6,12 months monitoring the following indicators:bone graft surgery time, intervertebral space height between vertebral activity corner, Japanese Orthopaedic Association (JOA) scores, Oswestry Disability Index, the fusion rate.Statistical data analysis using SPSS20.0 software.Result:The treatment effect of two Cage bone graft combined with pedicle screw internal fixation system in the treatment of lumbar instability is obvious,in the aspects of intervertebral space height, intervertebral active domain, JOA score, Oswestry Disability Index,the surgical treatment of two groups than the preoperative had significant improvement (P<0.05).Showed that the efficacy of Surgical treatment of lumbar instability is the exact.The fusion rate was observed from the two groups, two groups in the success rate of lumbar fusion without statistically significant difference (P> 0.05).Showed that two groups of operation treatment in the fusion rate had no difference.But,differences still exist between the individual observed indicators. Maintain the height of intervertebral space in the postoperative,in front of Cage intervertebral space impaction bone grafting was higher than that of the pure Cage bone graft group (P< 0.05);But in the operation of bone grafting time,A group was significantly less than in front of Cage intervertebral space impaction bone grafting group (P<0.05).Conclusion:The treatment effect of two Cage bone graft combined with pedicle screw internal fixation system in the treatment of lumbar instability is obvious.Although the two groups have no significant difference in the fusion rate,the two Cage bone graft fusion also exist differences:In front of Cage intervertebral space impaction bone grafting group could more effectively maintain intervertebral height:In front of Cage intervertebral space impaction bone grafting can be more conducive to fusion.Because it increased grafting bone mass and expanded the contact area with the superior and inferior endplate;simple Cage posterior interbody fusion has more advantages in saving operation time.
