A Preliminary Study On The Relationship Between Etiology And Pathogenesis Of ARDS In Traditional Chinese Medicine

Acute lung injury（ALI）/Acute respiratory distress syndrome（ARDS）is acommon clinical disease.It has numerouscauses, complex pathogenesis and highmortality,and it has gradually become one of important factors of death in ICU.It is considered in western medicine that ALI/ARDS is caused by theactivation of kinds of effector cells,such as pulmonary cpillary endothelialcells、alveolar macrophage、neutrophils and so on,along with the inflammatorymediatorsn and cytokine they release,which have a network effect in lung.There is no record of ALI/ARDS in Traditional Chinese Medicine(TCM).But itspathogenesis can be considered as” lung Qi upward reversal” depends on itsclinical manifestation,so we can treat it with the guide of a series of diseasessuch as “Cough”、” Dyspnea”, the pathogenesis of wich are” lung Qi upwardreversal”.In the medical classic of the Yellow Emperor,it has been recorded severaltimes that “cough is related to lung”.At the same time, it also has been recordedthat “cough is related all the tnternal organs of the body”.This is proceedfrom the the whole concept of the theory of traditional Chinesemedicine,descripting that though cough is a kind of disease occurring in lung,pathological changes of the other internal organs can also affect thelung,leading to the dispersive and descending function of lung, Lung qi adverseand cough.This passage depends on the classical theories in TCM,studying therelationship between the etiology and pathogenesis in TCM of ALI/ARDS and thetheory “cough is related to all the tnternal organs of the body”,consideringthat the lung is the main zang in ALI/ARDS,at the same time the heart failingto transform blood,the spleen and stomach failing to govern transportation andtransformation, the liver’s reverse restriction to lung,the kidney failing toreceive qi,the large intestine failing to transform, the riple energizer failingto the dysfunction of qi in transformation,which all can react on the lung,leading to the complicated condition of ALI/ARDS,with poor prognosis.
