Clinical Evaluation Of Lianlong Xiaoji Fang Combined With Elemene In The Treatment Of Advanced Primary Liver Cancer

Objective:To observe the curative effect of combined treatment with Lian-Long-eliminating-tumor-decoction and TACE with Elemene on the patients with advanced PHC, and to explore a more effective treatment for PHC.Methods:Using the open controlled clinical trial method,55 cases of advanced PHC were divided into two group:25 cases in treatment group and 30 cases in control group. The patients of contorl group received conventional TACE. In treatment group, Elemene was used as a perfusion or embolization agent during TACE and the LianLong-eliminating-tumor-decoction was taken orally and constantly through the full course of treatment. The following indicators were observed pre and post the treatment:the size of solid tumor, blood routine (WBC, NE%, HGB, PLT), AFP, liver and kidney function (ALB, ALT, AST, BUN, Cr), Child-Pugh score, the TCM syndrome integral, the clinical symptoms and the KPS. All of the data were statistically analyzed.Results:(1)The curative effect of solid tumor:the RR of treatment group and control group were 20.00% and 13.33%, and the CBR were 96.00% and 83.33%. The difference of curative effect of solid tumors had no statistical significance between the two groups (P>0.05)(2)The AFP levels:there was a statistical significance in difference between the AFP levels pre and post treatment in all cases (P<0.05), but no statistical significances in both groups (P>0.05). In treatment group, 20 cases’AFP levels changed,12 cases dropped and 8 cases rised, and in control group,21 cases changed,14 cases dropped and 7 cases rised. The difference of the drop ratio between the two groups had no statistical significance (P>0.05)(3)Liver function:in the treatment group, the differences of ALB, ALT and AST levels between pre and post treatment had no statistical significance (P>0.05). In the control group, the ALB and ALT levels decreased significantly (P<0.05),and the difference of AST levels had no statistical significance between pre and post treatment (P>0.05)(4)Child-Pugh scale:in both groups, the differences of the Child-Pugh scale between pre and post treatment had no statistical significance (P>0.05). In treatment group,25 cases’Child-Pugh level returned to the preoperative level, and 27 cases’did in the control group. The recovery ratio of the two groups were 100.00% and 90.00%,and the difference of the recovery ratio had no statistical significance between the two groups (P>0.05)(5)TCM syndrome integral:there were statistical significances in difference between the TCM syndrome integral pre and post treatment in both groups (P<0.05). The integral significantly decreased more in treatment gourp than in control group (P<0.05). The improvement ratio of TCM syndrome was 68.00% in the treatment group and 33.33% in the control group, and the difference of the improvement ratio had a statistical significance between the two groups (P<0.05)(6)Clinical syptoms:both groups had significant improvement in hypochondrium pain and abdominal distentionpain (P<0.05). Treatment group can significantly improve chest deep sigh, anorexia, loose stool, lassitude, dark complexion, dry stool, bitter taste, dry mouth and short yellow urine (p<0.05), but control group can’t (p>0.05)(7)KPS scale:in treatment group,the KPS scale increased significantly after the treatment, and the difference between pre and post treatment had a statistical significance (P<0.05). In control group, the difference of KPS scale between pre and post treatment had no statistical significance (P>0.05).The KPS scale significantly increased more in treatment group than in control group (p<0.05).The difference of improvement had a statistical significance between the two groups (p<0.05).(8) Security analysis:in treatment group, the differences of WBC, NE% and HGB levels had no statistical significances between pre and post treatment (p>0.05) but the differences of PLT level had (p<0.05). In control group, the difference of NE% level had no statistical significance between pre and post treatment (P>0.05) but the differences of WBC, HGB and PLT levels had (p<0.05). The difference of BUN and Cr levels had no statistical significance between pre and post treatment in both groups (P>0.05).Conclusions:Combined treatment with Lian-Long-eliminatingtumor-decoction and TACE with Elemene had no difference with pure TACE in the curative effect of solid tumor and the improvement of AFP levels, but had better effect than pure TACE in recovering liver function, shorteniing the recovery time of postoperative liver function damage, improving the clinical symptoms and physical condition, and had better security.
