Background:in recent years, more and more attention has been paid to health system for the rapid increase of medical fees. It is significance that public hospitals carry its reform by enhancing internal management, using limited resource in maximum, guaranteeing medical quality, promoting service efficiency, easing clinic process, and limit medical spending. Average length of stay (ALOS) is a comprehensive index to reflect the hospital operating efficiency. Reduce the average length of stay is the inevitable requirement of the reform of public hospitals, but the internal management of the hospital surviving the drawbacks of the traditional experience management model, the new management model and philosophy to play a management tool in the study of the effect of shortening the average length of stay is not yet over. For all these reasons, this paper provides some theoretical basis for how to shorten ALOS reasonably so as to raise operational efficiency.Aims:to comprehend the status quo of international ALOS study and relative index, and to study how to shorten ALOS by change of management model and idea.Methods:The subject tries to start from the internal management of the hospital, three general hospitals in Shanghai as the object of study, the collection and collation of statistical reports of hospital medical business, through questionnaires and expert interviews, the use of Excel and SPSS13.0statistical software to identify the impact of the hospital average the main factors affecting length of stay and hospital urology case analysis departments to strengthen internal management initiatives and effectiveness for the hospital to further reduce the average length of stay to provide references.Results:Health care system and health care policy, medical expenses and health insurance factors, patients and hospital factors, and technological factors are the main factors of the average length of stay. In the internal management of the hospital, the diagnosis and treatment process factors, the level of treatment technology factors, medical regulations factors are the main factors that affect the average hospital length of stay. Through the establishment of the outpatient ward "management model, to explore and establish the best treatment model, the use of advanced medical technology and equipment to speed up the construction of the medical information of running processes, improve clinical pathways, day surgery clinic system initiatives, can promote the hospital to further rationalize the average length of stay decreased. Conclusions:the bottleneck in hospital stay management can be broken effectively by enhancing internal management, applying new management mode and idea, optimizing and modifying of workflow by information. This study put its emphasis on how to shorten ALOS and increase operational efficiency in hospital by changing management model and renewing management methods.
