Obsessive-compulsive disorder(OCD),as a kind of intractable mental disorder that seriously affects the generation of combat effectiveness,poses a great threat to the mental health and even combat effectiveness of our soldiers.The occurrence of OCD in our military officers and soldiers has the following characteristics :(1)Difficult early identification.The difficulty of identification in primary health institutions may be related to the high heterogeneity and comorbidity of the disease itself.However,the military currently lacks the tools for early screening and effective assessment of compulsive susceptibility.(2)High incidence.According to the 2007 report on mental disorders of the PLA,the annual prevalence of OCD was 1.25 percent,the highest among 28 categories of mental disorders(single disease)in the residence survey,and the lifetime prevalence was 2.30 percent.That is to say,about 46,000 people in the PLA are staying in the troops,which brings many problems to the security and stability of the troops as well as the execution of military tasks.(3)Decline in job performance.Avoiding missions for fear of certain situations or scenarios,poor job performance caused by the compulsive checking,interpersonal tensions caused by compulsive behavior,even for depression violent conflict even suicide attempts(1/4),etc.,the existence of these problems has brought great influence to the patient’s life,work and study,and also brings great harm to the patient’s family.(4)Poor treatment effect.The treatment of OCD is challenging and may be related to the diversity of symptoms and the persistence of patients’ dysfunctional beliefs.Faced with such a recognized disabling disease,our troops generally have little awareness,and early detection,early diagnosis and early treatment is basically out of the question.The purpose of early identification research on high-risk groups of OCD is to provide methods and basis for the selection of military personnel,to screen out those with obsessivecompulsive personality before enlistment,to minimize the incidence of OCD among officers and soldiers,and to improve the overall psychological quality of new recruits and cadets.Research purpose:With OCD population at high risk as the research object,through the systematic literature review,compile specific personality scale peculiar to OCD,complete validation of OCD for screening high-risk groups,set up the CAT screening system for people at high risk for OCD,based on personality test,explore the eye movement characteristics of the high-risk groups of OCD,and build recognition model,through rational screening strategy,implementation of OCD high-risk groups early recognition and evaluation.Research object and method:This paper was divided into four parts,including 11 researches.The first part was the development and reliability and validity analysis of the subscales of the personality inventory for the high-risk population of OCD.The subjects of the study were soldiers aged 18-48,divided into three studies(study 1 to 3),which mainly compiled three subscales of the personality scale for high-risk population of OCD through standardized scale development procedures and reliability and validity analysis methods.The second part,the validity evaluation and total validation of the personality inventory for OCD high-risk population.Research object included military and local personnel,divided into four studies(study four to seven),mainly through structured interviews to explore the predictive validity of personality inventory in high-risk population with OCD,to evaluate the effectiveness of the OCD-personality scale via a third-party evaluation,by statistical comparison to discuss the OCD-patients and health people,high-risk groups of the differences in personality scale score.The seventh study was to verify the effectiveness of the personality scale through the practice of psychological selection of civilian personnel in military recruitment.The third part,the CAT study of the personality scale of high-risk population of OCD.The subjects were soldiers aged 18-48,divided into two studies(Studies 8-9),including simulated CAT study and real CAT system development.The fourth part,OCD high risk population personality test eye movement characteristics and recognition model.The subjects were soldiers aged 18-48,including two studies(study10 to 11),which mainly discussed the eye movement characteristics of high-risk population with OCD under the stimulation of personality test,as well as the valuable optimization function of eye movement in the identification model and screening strategy of people at high risk of OCD.Research results:The first part: Compilation and reliability and validity analysis of three subscales of obsessive-trait,obsessive-belief and obsessive-metacognition.Among them,there were 53 items of 4 factors in the obsessive-trait scale,36 items of 3 factors in the obsessive-belief scale,and 26 items of 3 factors in the obsessive-metacognition scale.The main results were as follows :(1)The consistency coefficients of content validity of each subscale were >0.90;(2)The results of factor analysis and correlation analysis showed that the structure of each subscale complied with the theoretical hypothesis,and TLI,CFI,RMSEA and WRMR all met the requirements of psychometrics.(3)The Cronbach α coefficient of each subscale was >0.90,and the composite reliability and test-retest reliability met the requirements of psychometrics.Part II: Effectiveness evaluation of personality scale for high-risk group of OCD.Main results were as follows:(1)The results of structured interview showed that the predicted coincidence rate(e.g.,positive and negative predictive value)of high-risk group and control group were both >0.90,and the total predicted coincidence rate was >0.90;(2)The results of third-party evaluation showed that the positive rate of high-risk group was significantly higher than that of control group(χ2=30.259,p<0.01).(3)The results of inter-group comparison showed that there were significant differences in the obsessive-trait subscale between the high-risk group and the patient group(t=-1.997,p=0.046);The "sensitivity" of personality scale for high-risk group of OCD reached 0.92.Part III: Research on Computerized Adaptive Testing of personality scale screening.Main results were as follows :(1)PC and network versions of the personality screening CAT detection system for high-risk people with OCD were developed;(2)After two rounds of parallel measurement with 80 and 200 people,the results showed that the system runs smoothly and efficiently.The correlation coefficient with the CTT screening results was0.79,the average number of test items was reduced by 51.5%,and the average test time was shortened by 49.6%.All the indexes can meet the requirement of psychometrics.Part IV: Study on eye movement characteristics and identification model construction of high-risk people with OCD.The main results were as follows :(1)The duration of fixation in the item-option area was higher in the high-risk group than in the control group(t=3.02,p <0.01);In terms of fixation time in item-stem area,the high-risk group was smaller than the control group(t=-1.863,p <0.01);(2)The saccade amplitude of the high-risk group was higher than that of the control group(t=2.210,p=0.030).(3)Through the establishment of an identification model and the formulation of multi-dimensional screening strategies,the study confirmed that the accuracy of the CAT system in identifying high-risk population with OCD was more than 92%.Main research conclusions:1.The reliability and validity indexes of the three personality screening subscales for high-risk population of OCD developed in this study.All met the psychometric requirements;2.Through structured interview,third-party evaluation,three groups of population comparison and practical psychological selection,it is proved that the high-risk population personality scale has an acceptably high empirical validity,and can effectively prevent the missed screening and mis-screening caused by a single trait scale.3.CAT of the personality scale for high-risk population of OCD significantly improved the test efficiency and validity of the scale;4.There were significant differences in eye movement characteristics between the highrisk group and the normal control group on the personality scale of OCD high-risk group,which could be used as an auxiliary calibration index for personality scale screening.5.The effectiveness of the identification model and multidimensional screening strategy for high-risk patients with OCD is greater than that of traditional single-trait screening.Research innovation point:1.The personality structure of high-risk group of OCD was proposed,and the personality scale of high-risk group of OCD was developed,and the total prediction coincidence rate was 91.43%;2.For the first time,the CAT of personality screening for high-risk people with OCD was implemented,and a fast discriminant method to distinguish the dominant process and the ideal point process was put up,which significantly improved the efficiency and validity of personality test;3.The eye movement characteristics of high-risk population with OCD were found under personality inventory,and a recognition model of the fusion of personality test’ response and eye movement were established to improve the accuracy of screening.4.A test-retest reliability analysis method based on latent state-trait model is proposed to improve the scientific nature of reliability measurement. |
