Objective:To construct a model of traditional Chinese medicine(TCM)combined with chemotherapy for the treatment of advanced breast cancer(ABC)by combining the thought of"survival with tumor",the thought of" preventive treatment of disease " and the basic principles of game theory,so as to provide treatment ideas and decision-making reference for the treatment of ABC involving TCM under the mode of systematic thinking.Methods:The cancer management model of ABC was established by system engineering methodology,and the treatment process of ABC was analyzed from the perspective of system engineering by literature analysis,mathematical derivation,equation based modeling and multi-agent simulation.According to the research idea of Hall 3D structure,the treatment process of TCM combined with chemotherapy for ABC was summarized into three dimensions:logical dimension,professional dimension and time dimension for analysis.In the logic dimension,in order to determine the treatment goal of ABC,the idea of "survival with tumor" and evolutionary game theory were combined to construct the "survival with tumor" model based on evolutionary game theory.In order to explore intervention strategies,the idea of"preventive treatment of disease "was combined with the Stackelberg game to construct an intervention model for ABC based on the Stackelberg model.In terms of professional and time dimension,the "survival with tumor" model and the " preventive treatment of disease"model were simulated by the multi-agent simulation method to verify the status and feasibility of the two models in actual operation.Results:1.By establishing the cancer management model of ABC based on WSR and Hall 3D structure,the problems faced by cancer management of ABC were analyzed from multiple dimensions.At the macro level,the system thinking is used to analyze Wuli,Shili and Renli.At the micro level,the cancer management framework of ABC is based on Hall 3D dimensional structure model.The system engineering methodology is adopted to provide theoretical guidance for the cancer management of ABC.At the macro level,the paper proposed the application of the WSR-based biopsychosocial medicine model to the field of ABC management.The paper proposed to recognize the problems of ABC management from three aspects:biological,psychological and social,and to break down the process of ABC management.At the micro level,the Hall 3D structure is used to outline the stages,steps and scope of knowledge involved in the management of advanced breast cancer from the perspective of a three-dimensional spatial structure consisting of temporal,logical and professional dimensions.The cancer management model of ABC based on WSR and Hall 3D structure was established,and the scientific thought system of ABC management system supported by the methodology of combining soft and hard systems was formed.2.By establishing the mathematical model of "survival with tumor" as the treatment goal of ABC,it is clear that the realization of "survival with tumor" is to control tumor load and prolong survival by controlling tumor cell population and maintaining tumor subclone ratio through ESS.Using the modeling method of evolutionary game theory,the concept of "survival with tumor" was embodied as a "hawk dove game".It is clear that the treatment methods of TCM are involved in the treatment process of ABC with the goal of "survival with tumor" by improving the existing symptoms,reducing dose toxicity and killing certain tumor cells.It was proved that the method of "survival with tumor" is to inhibit the total number of tumor cell population and control the proportion of drug sensitive subclonal cells,and the growth of drug resistant subclonal cells by relying on the pressure of the competitive growth of its own population,so as to reduce the occurrence of drug resistance and prolong the survival of tumor.3.Through the simulation analysis of the "survival with tumor" model,the feasibility of"survival with tumor" as the therapeutic goal of ABC was proved.In order to explore the optimal therapeutic target of ABC,the effect of the competition between heterogeneous tumor subclones on the tumor population system was analyzed through the simulation analysis of the"survival with tumor" model.In the absence of external intervention factors,the number of each subclonal cell is only related to the game coefficient between the subclonal population when the tumor population is finally stable,and the system shows an asymptotically stable ecological cycle,and the population ratio at the stable time is independent of the initial ratio of each subclonal cell.By understanding the proportion of two kinds of subclones in the population and carrying out agent-based simulation analysis,the state of population equilibrium can be understood,the evolutionary direction of the population can be mastered,and the direction of intervention can be clarified.By controlling the proportion of the population,the overall number of the population can be controlled and the goal of "survival with tumor" can be achieved.4.Based on the idea of "preventive treatment of disease" in TCM,a Stackelberg game model is constructed,and satisfactory strategies of drug intervention in the treatment of ABC can be obtained by solving the equation.To study the best intervention strategy in the treatment of ABC,and the paper combined the idea of "preventive treatment of disease" combine the principle of game theory,by using the modeling based on the equation of a Stackelberg game model.This model could take advantage of the doctor "leader" status in the game,effectively balance the tumor burden in the process of ABC treatment and dose toxicity.Using backward induction method to solve the subgame Nash equilibrium,the optimal solution of the treatment strategy can be obtained when the doctors benefit the most.The model also proved that the strategy of treating ABC with TCM combined with chemotherapy drugs can reduce the impact of drug dose toxicity on doctors' benefits,further improve doctors' benefits,and have important practical significance for improving the prognosis of ABC.5.Simulation based on ABM was used to verify the advantages of chemotherapy strategy based on game theory and chemotherapy strategy combined with TCM.In order to explore the effects of different intervention methods and intervention opportunities on the treatment of ABC with TCM combined with chemotherapy,the game theory was used to optimize the treatment strategies of TCM and western medicine for ABC.An ABC tumor population composed of two kinds of subclones was established by using ABM simulation,and the simulation results showed that the development process of tumor could be simulated well by using ABM simulation,and the chemotherapy regimen based on game theory could not only reduce the dose of drug,but also prolong the process of drug resistance of tumor.The chemotherapy strategy combined with TCM can significantly reduce the number of administration and reduce the tumor load,which has great practical significance for the clinical treatment of ABC.Conclusion:This paper applied the system engineering methodology to analyze the problems in the treatment of ABC from multiple dimensions,innovatively adopted game theory to provide theoretical guidance for the management of ABC,and gave full play to the characteristics and advantages of TCM theory and thought in the treatment of ABC.The establishment of the model of TCM combined with chemotherapy for the treatment of ABC is conducive to the scientific and reasonable design of treatment strategies for ABC,and has great theoretical and practical significance for the treatment of ABC.
