Objective:The clinical study was to observe the clinical efficacy of Shuimu H ening Prescription combined with Paroxetine in the treatment of Parkinson's dis ease with depression.Experimental studies are based on the autophagy-lysosom al pathway to explore the mechanism of Shuimu Hening Prescription on Parkin son's disease.Methods:Clinical study:96 patients with Parkinson's disease with depression t hat meet the criteria were randomly divided into a test group and a control gr oup,with 48 patients in each group.Collect the patient's age,gender,course o f disease,H-Y classification and other data for statistical analysis.The experim ental group was treated with Shuimu Hening Prescription,Madopar and Paroxet ine,while the control group was treated with Madopar and Paroxetine only.Af ter 4 weeks and 8 weeks of treatment,the TCM syndrome score,HAMD scale score,UPDRS-III scale score,and PDQ-39 scale score were statistically analy zed.Experimental study:Use MPP~+to act on PC12 cells to construct a PD ce ll model,and use the MTT method to screen the optimal concentration of MP P~+,Shuimu Hening Prescription,and Madopar.Flow cytometry was used to det ect the apoptosis of PC12 cells in the control group,MPP~+model group,Shui mu Hening Prescription+MPP~+group,and Madopar+MPP~+group.Western blot and RT-PCR methods were used to detect the expression of?-syn,TH protein and its m RNA,and ALP-related P62,Beclin1,LC3-II protein and its related m-RNA.Results:Clinical study:The clinical trial finally enrolled 86 patients,43 in eac h group.1.The gender,age,course of disease,past history,education level an d other data of the test group and the control group were not statistically signi ficant through relevant statistical analysis(P<0.05),that is,the test group and t he control group are comparable.2.After 4 weeks and 8 weeks of treatment i n the test group and the control group,the HAMD scale scores,UPDRS-III sc ale scores,and PDQ-39 scale scores of the two groups of patients decreased,a nd the test group was better than the control group(P<0.05),and the HAMD curative effect of the test group is better than that of the control group.2.TC M syndrome scores:After 4 weeks and 8 weeks of treatment,the TCM syndr ome scores of the two groups of patients decreased,and the test group was be tter than the control group(P<0.05),and the clinical efficacy judgments of the two groups showed that the test group was excellent in the control group(P<0.05).3.There were no adverse reactions in the two groups of patients during t he medication,and Shuimu Hening Prescription are safe.Experimental study:1.The optimal concentration of MPP~+induced PC12 cells to construct PD mode l by MTT method is 1m M,the optimal protective concentration of Shuimu He ning Prescription is 6mg/ml,and the optimal protective concentration of Madop ar is 3.125ug/ml.2.Shuimu Hening Prescription and Madopar can promote the survival of PC12 cells induced by MPP~+,while protecting their cell morpholo gy.3.The optimal protective concentration of Shuimu Hening Prescription and Madopar has the effect of antagonizing the apoptosis of MPP~+on PC12 cells,and the anti-apoptotic decline of Shuimu Hening Prescription is better than that of Madopar group.4.Western blot and RT-PCR test results show that Shuimu Hening Prescription can down-regulate the expression levels of?-syn,P62 pro tein and their m RNA,and increase the expression levels of LC3-II,Beclin1,T H protein and their m RNA,while Shuimu Hening Prescription have an effect on the regulation of?-syn,P62,Beclin1,TH protein and m RNA is better than Madopar,but the regulation of LC3-II protein and its m RNA is not much dif-ferent.Conclusion:1.Shuimu Hening Prescription combined with paroxetine in the tr eatment of Parkinson's disease with depression has a definite clinical effect co mpared to paroxetine alone.It can reduce the TCM syndrome score,HAMD s core,UPDRS-III score and PDQ-39 score of patients to a certain extent,and i mprove the overall quality of life of patients with clinical safety and reliability.2.Shuimu Hening Prescription may promote the degradation of?-syn by regul ating the ALP mechanism,thereby protecting dopaminergic neurons and having anti-apoptotic effects.
