Objectives:The purpose of this study is to explain the minimally invasive surgical techniques of primary retroperitoneal tumor resection,to evaluate the safety and effectiveness of the surgery,to analyze the indicators of the perioperative outcome and short-term prognosis to see if it is beneficial.Methods:A retrospective analysis of 76 cases of open retroperitoneal tumor surgery and 51 cases robot assisted surgery were carried out.The perioperative data were observed and analyzed,including the size of the tumor,the time of operation,etc.Also,for retroperitoneal tumor adjacent to large vessels,data of 24 cases of open surgery and 17 cases of robotic surgery were analyzed with the same method.Results:The average operation time of the robotic group was shorter than that of the open group(117.4±6.6min vs 203.1±7.7 min),and postoperative drainage time was shorten(3.02±0.34 d vs 7.61±0.91 d,p=0.0001).The open diet time was also shortened(4.41±0.33 d vs 6.58±0.67 d,p=0.0130),and hospital stay was significantly shortened(14.24±0.83 d vs 20.71±1.59 d,d p=0.0021).For tumor adjacent to major vessels,the average operation time of the robotic group was shorter than that of the laparoscope group(121.5±14.27min vs 177.9±13.36min,p=0.0072),and the average blood loss was smaller(74.12±21.25ml vs 410±136.3ml,p=0.0477).The postoperative drainage time was shorter(3.53±0.77d vs 5.33±0.31d,p=0.0133),and the open diet time was shorter(3.41±177.9d vs 4.71±0.39d,p=0.0212).Conclusion:Robot-assisted retroperitoneal tumor resection can achieve similar surgical results and safety as laparotomy,and the advantages are mainly manifested in the rapid recovery after operation,and the objective of minimally invasive treatment of retroperitoneal tumor is achieved.For tumor adjacent to major vessels,compared with laparoscopic group,the main advantage of the robotic retroperitoneal tumor surgery is to improve the efficiency and efficacy of the operation,and the postoperative recovery can also achieve same ideal outcome.
