Purpose:The obesity,T2 DM and MS have the same pathological basis,and the basis is insulin resistance.The clinical reasearch that The Yunfu-Tongjing Massage could cure the above diseases,but its mechanism is not clear.According to the Theory of "Zangfu-Jingluo related" in Chinese medicine,We assume that the mechanism is associated with energy metabolism and conduction,especially with the SIRT1/PGC-1? signal pathway,to reveal the Yunfu-Tongjing Massage on the mechanism to the IR of obesity T2 DM.Methods:1.Adopting "High-Fat and High-Sugar feed combined with low dose STZ intraperitoneal injection",establish the rat model of IR for obesity T2 DM.After the success of the model,50 model rats were randomly divided into the model group,the control group,No.1massage group,No.2 massage group,No.3 massage group.In the same period,10 rats were purchased for normal feed,and 10 were normal group.Treatment 5 days per-week and keep 4weeks.Observing the general state of rats in each group and calculate the Lee's index.2.The rat was inspect FBG? TC?TG?HDL-C?LDL-C? FINS?LP? TNF-??IL-6?IRI?ISI thought the ELISA.3.HE stain was used to observe the morphological changes of skeletal muscle,adipose tissue and pancreas in rats.4.The rat was detected the levels of AMPK,SIRT1 and PGC-1? protein and gene expression associated with signal pathway thought Western Blot and RT-PCR.Result:1.The generally results showed that:(1)After 3 weeks intervention,there was no significant difference in food intake between No.2 massage group,No.3 massage group and the normal group(P>0.05),suggesting that massage may have mild suppress appetite.(2)After4 weeks intervention,There was significant difference in body weight between No.2 massage group and No.3 massage group?control group(P<0.05).It is suggested that massage has thefunction of reducing weight,and the manipulation effect of medium operation time is better than the other two groups.2.The ELISA results showed that:(1)Compared to the model group,After 4 weeks intervention,The FBG levels of the control group,No.2 massage group,No.3 massage group were significantly decreased,showing a significant difference(P<0.01).The results of LSD showed :There were no significant difference between the control group and No.2 massage group and No.3 massage group(P>0.05);There was no significant difference between No.2massage group and No.3 massage group(P>0.05).It is suggested that Yunfu-Tongjing massage can reduce FBG levels,and the effect is similar to that of the control group.(2)After 4weeks of intervention,compared with the model group,the FINs level of the control group,No.2 massage group and No.3 massage group decreased significantly,showing an extremely significant difference(P<0.01).The results of LSD showed : There were significant differences between No.2 massage group and control group(P<0.05);no significant difference between between No.3 massage group and control group(P>0.05);There was a significant difference between No.2 massage group and No.3 massage group(P<0.01).It is suggested that the use of insulin can be promoted and the compensatory secretion can be improved.And the curative effect of No.2 massage group is better than the control group.(3)Yunfu-Tongjing massage can improve insulin resistance index,increase insulin sensitivity and reduce body weight.(4)Compared to the model group,After treatment,the levels of TC,TG and LDL-C in the control group,No.2 massage group and No.3 massage group were significantly decreased.while the levels of HDL-C and FFA increased,showing an extremely significant difference(P<0.01).The levels of LP,TNF-?,and IL-6 were significantly decreased,showing an extremely significant difference(P < 0.01).It was suggested that proper operation time of Yunfu-Tongjing massage can improve lipid metabolism and inflammation in rats.3.The HE staining results showed that(1)The number of skeletal muscle cells was normal in the normal group,and the muscle fibers were long and parallel arranged.In the model group,the muscle fibers were out of shape and the muscle space was narrow.The No.1massage group of skeletal muscle cells was similar to that of model group.The skeletal muscle cells of the No.2 massage group,No.3 massage group and the control group were in good shape and the muscle gap was widened,but the muscle gap was not restored to thenormal group level,and there was no significant difference between the three groups.(2)In the normal group,the fat around the kidney was full in shape,uniform in area and clear in boundary.The adipocytes in the model group were small and full,the number of adipocytes increased,and the average area was less than that in the normal group.The size and shape of fat cells in No.1 massage group were similar to those in the model group.The adipocytes of the No.2 massage group increased in area and the number of adipocytes in visual field decreased compared with the normal group.The fat cells of the No.3 massage group was close to the control group,and the size of the fat cells was between the normal group and the model group.(3)The islet cells of the normal group were regular in shape,oval or round in shape,with a large number of cells and nuclei in the islet and clear boundaries.The pancreatic islet cells in the model group and the No.3 massage group were disordered in structure,irregular in shape,smaller in pancreas islet area,and fewer cells and nuclei in pancreas islet.The atrophy of pancreatic islet cells in the No.2 massage group was significantly improved compared with the control group and the No.1 massage group.The overall shape and area of pancreas islets in No.1 massage group and control group recovered to some extent,but the effect was not as good as that in No.2 massage group.4.The Western Blot results show that :Compared with the model group,the expression levels of AMPK,SIRT1,PGC-1 ? in the control group,No.2 massage group,and No.3massage group were significantly improved,showing extremely significant differences(P<0.01).There was no significant difference between the No.1 group and the model group(P>0.05).It is suggested that Yunfu-Tongjing massage can improve protein expression level of the SIRT1/ PGC-1? signal pathway,improve insulin resistance.5.The RT-PCR results showed that:Compared with the model group,the expression levels of AMPK,SIRT1,PGC-1 ? in the control group,No.2 massage group,and No.3massage group were significantly increased,showing extremely significant differences(P<0.01).There was no significant difference between the No.1 group and the model group(P>0.05).It is suggested that Yunfu-Tongjing massage can improve mRNA expression level of SIRT1/ PGC-1? signal pathway,improve insulin resistance.Conclusion:1.There is a "time-effect relationship" between the manipulation of massage,and the medium operating time is superior to other groups in many indicators,showing good clinical effects,which is of certain reference significance for clinical formulation of massage prescription.2.The medium operation time of Yunfu-Tongjing massage can promote the expression of lipid inflammatory factors,adjust lipid metabolism,and then improve insulin resistance.3.The medium operation time of Yunfu-Tongjing massage can improve the content of skeletal muscle cells in model rats,reduce the number of fat cells,promote the repair of islet cells,and thereby regulate energy metabolism.4.The medium operation time of Yunfu-Tongjing massage may be through improved model rat inflammatory state,regulating lipid metabolism,and activate the skeletal muscle AMPK,promote SIRT1 protein and mRNA expression level,and increase the downstream signal factor PGC-1 ? protein and mRNA level,namely yun abdominal menstruation may through the SIRT1 / PGC-1?signal pathway to improve insulin resistance.
