Background and Objective:In recent years,the number of elderly patients with hyperuricemia and osteoporosis in China is gradually increased.Oxidative stress is the basic mechanism for the reduction of bone mass in age-dependent osteoporosis.Previous studies have confirmed serum uric acid(UA)is the final enzyme product of sputum metabolism.UA,as an antioxidant,can be acted as an antioxidant through its free radical scavenging ability.It has a physiologically beneficial effect.Blood uric acid may play a beneficial role in bone mineral density(BMD)and bone metabolism,by means of antioxidant mechanism.Therefore,in order to study the relationship between the two factors,this study aims to explore the correlation between blood uric acid and femoral neck bone mineral density and related differential proteome changes in patients with hyperuricemia.The research will provide experimental and clinical evidence for the prevention and treatment of osteoporosis in elderly patients with hyperuricemia.Methods:The clinical data were divided into male and female groups according to gender.Each group was divided into UA group(high blood uric acid)and control group(normal blood uric acid)according to serum uric acid.Clinical characteristics(sex,age,uric acid,bone density,coagulation parameter,D-dimer)data were processed by correlation analysis.The femoral neck bone tissue materials of hip replacement surgery patients were collected.The differential proteins between different groups were screened by Label-free mass spectrometry,and the sequence,structure and function information of the protein was obtained by database search.Finally,the KEGG function was performed and analyzed.The material sample of tissue,with the help of the levels of peroxisome proliferator-activated receptor(PPARy)and PLIN1 protein expression were detected by immunoblotting(WB).The obtained experimental data were expressed as mean±standard deviation(x±s).Statistical analysis was performed by SPSS 19.0 software.ANOVA one-way analysis of variance was used for comparison between groups,and the statistical significance was set to P<0.05.The correlation between the age,femoral neck BMD and UA was analyzed by Spearman rank correlation coefficient,P<0.05,and the two variables were considered to be related.Result:(1)The mean blood uric acid concentration and average femoral neck BMD were higher in men than in women(P<0.05).There was no correlation between age and uric acid in men and women(P>0.05),and there was a negative correlation between age and femoral neck BMD(r<0,P<0.01).In men,uric acid was not significantly associated with bone mineral density.While in women,blood uric acid was positively correlated with femoral neck bone density(r>0,P<0.01).Stereoscopic observation showed that the trabecular arrangement of the UA group was denser,the spacing was smaller,the connection was increased,and the texture was clearer.The trabecular bone arrangement of the control group was sparse.The spacing was widened,and the texture was uneven.In the same sex,the UA group was small.The percentage of beam area(%Tb.Ar)was significantly different from the control group(P<0.05).Examination of the intersubject effects of PT,INR,APTT-FIB,and D-dimer in the UA group and the control group showed no statistical difference between the two groups(P>0.05).(2)A total of 66 differential protein spots were selected to meet the folder change>2 and the significance analysis P-value<0.05.The differential proteins were mainly involved in biological processes such as PPAR signaling pathway and fat metabolism.(3)WB showed that the levels of PPARy and PLIN1 protein in UA group were significantly different from those in the control group(P<0.05).The difference of PPARy and PLIN1 protein content in male UA group was different from that in female UA group,which showed a statistically significance(P<0.01).The PPARy results of immunohistochemical staining were consistent with the WB.Conclusion:(1)There is significent correlation between blood uric acid and femoral neck bone density in elderly women.Correlation between blood uric acid and bone mineral density is a result of comprehensive analysis of bone density,histology and pathology.(2)The femoral neck of patients with hyperuricemia contains a variety of differential proteomes.PPARy and its associated fat metabolism are biological pathways with significant differences.(3)The WB and IHC verification of PPARy and PLIN1 demonstrates the accuracy and reliability of mass spectrometry studies.PPAR? and PLIN1 are down-regulated in bone tissue of patients with hyperuricemia.
