| Objectives: The predisposing gene prediction model for asthma in Han children consists of 4 single-nucleotide polymorphisms(SNPs)including IL13 rs20541(R110Q),IL4 rs2243250(-590 C > T),ADRB2 rs1042713(R16G)and FcER1 B rs569108(E237G).We aimed to determine the association between the predisposing gene prediction model for asthma in Han children and childhood asthma,so as to evaluate the effect of the model in the application of the diagnosis and prediction of childhood asthma.Methods: Wheezing children aged between 6 months and 5 years were enrolled among outpatients of Xinhua Hospital affiliated to Shanghai Jiaotong University during Dec,2014 and Mar 2015.High and low risk group were partitioned by the predisposing gene prediction model for asthma in Han Children.Clinical features of children with wheeze were described.We analyzed the association between the model and children with wheeze as well as risk factors for wheezing children with a positive asthma predictive index(API),and we evaluated the effect of the diagnosis provided by the model compared to Childhood Asthma Diagnostic Score in China and Diagnostic Criteria for Childhood Asthma in Canada.Results:(1)212 wheezing children were recruited in the study,and numbers of episode of wheeze,wheeze without colds,eczema,positive results of food and aeroallergens,eosinophilia were significantly associated with the predisposing gene prediction model for asthma in Han Children.(2)Single-factor analysis indicated that occurring age of the first episode of wheeze,numbers of episode of wheeze,allergic rhinitis(AR)and eczema,positive results of food and aeroallergens and high-risk result of the model were significantly associated with wheezing children with a positive API.Multivariate analysis indicated that occurring age of the first episode of wheeze before age of 2(0-1 years;1-2 years,OR=10.433),numbers of episode of wheeze before age of 3(OR=5.704),AR(OR=7.341),eczema(OR=9.405),high-risk result of the model(OR=65.722)were risk factors for wheezing children with a positive API.(3)The predisposing gene prediction model for asthma in Han Children is quite predictive(Sensitivity: 77.2%,Specificity: 80.0%,PPV: 74.7%,NPV: 82.1%,AUC: 0.786).However,its asthmatic prediction was lower than Childhood Asthma Diagnostic Score in China(Sensitivity: 96.7%,Specificity: 86.7%,PPV: 84.8%,NPV: 97.2%,AUC: 0.917),and the consistency level was of average(P=0.22,Kappa=0.49).The specificity,PPV and AUC of the model were higher than the Diagnostic Criteria for Childhood Asthma in Canada(Sensitivity: 97.8%,Specificity: 53.3%,PPV: 61.6%,NPV: 97.0%,AUC: 0.539),and the consistency level was not high(P<0.01,Kappa=0.265).Conclusions: The predisposing gene prediction model for asthma in Han children composed by 4 SNPs including IL13 rs20541(R110Q),IL4 rs2243250(-590C>T),ADRB2 rs1042713(R16G)and FcER1 B rs569108(E237G)was associated with childhood asthma,and was one of risk factors for wheezing children with a positive API together with occurring age of the first episode of wheeze,numbers of episode of wheeze,AR and eczema,and it can be applied as one of tools to assist in the diagnosis of childhood asthma due to its diagnostic and predictive value. |
