Study Expression Of LncRNA-HEIH Of Serum And Exosomes In HCV Related Hepatocellular Carcinoma

Objective:To investigate the expression and clinical significance of LncRNA HEIH in HCV related hepatocellular carcinoma;To explore the real world data on the treated chronic hepatitis C with Daclatasvir combined with Sofosbuvir,and to guide clinical treatment.Methods:We collected blood samples from 35 patients with chronic hepatitis C,22 cirrhosis patients and 10 hepatocellular carcinoma patients with hepatitis C related in Huzhou Central Hospital of Zhejiang Province.The serum levels of LncRNA-HEIH were test and their clinical significance was analyzed.At the same time,serum exosomes were extracted from 10 patients in each group.The ultrastructure of exosomes was observed by electron microscope.The levels of LncRNA-HEIH in exosomes was quantitatively test and analyzed about its clinical effect.We retrospectively analyzed 56 patients with chronic hepatitis C.All patients were treated with Daclatasvir combined with Sofosbuvir for 12 weeks and followed up again when completed treatment after 12 weeks.During the treatment,the CAP and LSM were measured by Fibroscan;and test HCVRNA,blood routine,liver and kidney function,blood lipids,etc.Patients should be test again for 12 weeks and after 12 weeks.We observed the virological response at 2,4,8 weeks and the end of treatment.Then detected the efficacy,safety and adverse reactions of liver and kidney function,blood routine,blood lipids;at last the real clinical data were analyzed.Results:The serum levels of LncRNA HEIH in patients with liver cancer and cirrhosis were higher than those in patients with chronic hepatitis C(P<0.05).The level of LncRNA HEIH in hepatoma exosomes was higher than that in cirrhosis group(P<0.05).The level of HCC group was also higher than that of chronic hepatitis C group(P<0.05).The expression of LncRNA HEIH in the exosome and serum of HCC was increased,but the ratio of exosome to serum was decreased.Variations in Y93H and L31M and HCV RNA level before and after treatment did not affect the sustained virological response.Antiviral therapy had significant effects on direct bilirubin,albumin,globulin,glutamyl aminotransferase,creatinine and uric acid(P<0.05).Following up for 12 weeks after treatment,the liver fibrosis of the patients was better than that before(P<0.05).There were fewer adverse reactions in the course of treatment.Age was an important factor affecting the occurrence of adverse reactions.Conclusions:The expression of LncRNA HEIH in serum and exosomes of patients with hepatitis C-related liver cancer was elevated,but the ratio of exosome to serum was decreased.SVR12 of Daclatasvir combined with Sofosbuvir in the treatment of chronic hepatitis C was as high as 98.11%.Antiviral therapy can improve liver fibrosis in patients with chronic hepatitis C.There were fewer adverse reactions in the course of treatment.Age was an important factor affecting the occurrence of adverse reactions.
