Background:Cosmetic reconstruction of thumbs and fingers,which refers to the complex splicing of tissue and flaps transplanted from different parts of the body of the patient with surgical design in order to assembly reconstruct the missing thumbs and fingers with a similar shape and function to the normal fingers.In this study,attention will be focused on the related technical problems in the technique of cosmetic reconstruction of thumbs and fingers,clinical anatomy study of the toe donor area will be carried on,study of the digital design combined with the design and implementation of the 3D print-assisted reconstruction surgery will be involved and put forward some new methods for the preparation of the artificial joint prosthesis.Objective:Intravenous refraction is an important factor for the success of the flap grafting to repair the defect of the fingers.This study will carry on an anatomical study on the geometric study of the hallux toe veins and put forward anatomical indications for the clinical application.The application of 3D printing technology was used to study on the design of the flap and the iliac bone graft in order to improve the effect of digital design of the flap and accuracy for the iliac bone graft.The morphological matching will be verified in order to qualify the morphological matching of the 3D artificial finger joints.Methods:Fresh foot specimens were stained with blue latex and then subjected to microscopic anatomy,and anatomical specimens were collected.Clinical cases were collected and observed and measured for the toe veins.3D printing technology combined with digital design were used to guide the design of donor site for tihe thumb reconstruction before surgery and guide the clinical operation.The iliac bone transplanted navigational block were designed in the CAD software combined with 3D printing technique in order to guide the resection of the iliac bone and reconstruct the length of the missing parts of the transplanted bones.The iliac navigation module was designed to guide the removal and shaping of the iliac bone.In this study,Geomagic Qualify were used to verify the design model,3D printing model and medullary cavity shape,and to provide the experimental basis for the design of personalized finger joint in the artificial finger joint.Results:1.There are two groups of veins,the craniofacial veins,the diameter is relatively thick and the position is relatively constant,which are suitable for clinical applications.The walking law of the toe dorsal veins is concentrated?decentralized?eonnected?redistribution,in the interphalangeal joint at the anatomical findings,few venous distribution exists,for which this area were called the desert of the toe veins;2.7 clinical cases of thumb reconstruction patients were carried out digital design before the surgery,and guide the surgical area for the cut,to achieve the design area for the precise,personalized selection purposes;3.For 10 patients with full-shaped resuscitation of the thumb,the iliac bone navigation module was designed to guide the iliac bone incision,which can reduce the operation time and improve the operation effciency.4.For the artificial finger joint intramedullary handle design,the design model,the 3D printined model and the shape of the medullary cavity were mostly matched.Conclusion:Anatomical study on the regularity and measurement of the vein of the great toe can provide anatomical reference for the clinical miniature flap removal.For the thumb finger reconstruction,with the digital design combined with 3D printing,reconstruction pre-supply area for the design of the defect part of the physical model reference can be achieved and optimal design for the donor area and precisely cut will be implemented.In view of the difficult technique of iliac bone transplantation in the cosmetic operation of the thumbs and fingers,the personalized iliac bone navigation template was designed,and the navigation module was printed with the 3D printing technology to realize the accurate cutting of the iliac bone.Based on the preliminary study on the morphological matching of individual artificial finger joint preparation methods,the matching of 3D fingerprints of artificial fingerprints was preliminarily verified,which provided an new idea for individual artificial finger joint preparation.
