Background: Stroke is a major consequence of cardiovascular disease(CVD).The onset of a stroke frequently results in permanent physical disability,decreased autonomy,cognitive impairment,depression,and death.Therefore,there is an urgent need to strengthen the prevention and treatment of stroke is to reduce the burden of disease and reduce the morbidity.Objective: The purposes of this study were to complete large-scale screening and assessment of high-risk individuals with stroke,and to observe the dynamic status of stroke in stroke-prone areas at a high risk of stroke in Urumqi.This study completed 2000 cases of high risk individuals with stroke intervention comparison.It attempted to provide more reliable and powerful evidence for cerebral infarction for clinical practice by a meta-analysis of Ginkgo-dipyridamole injection plus aspirin clinical randomized controlled trials.Methods: 1.Literatures concerning Ginkgo-dipyridamole combined with aspirin in the treatment of cerebral infarction published in PubMed,Web of Science,Chinese Academic Journal Full-text Database(CNKI),China biomedical literature database(CBM),and the Wan Fang database was retrieved and randomized controlled trials were selected according to the inclusion criteria.Meta-analysis was carried out using RevMan 5.3 software and the pooled risk ratio(RR)and standardized mean difference(SMD)of the main outcomes with the 95%confidence interval(95%CI)were estimated.And use GRADE PRO software to evaluate the effectiveness of evidence.2.The study screened by questionnaire from September 2015 to December 2017,Xinjiang Medical University Health Management Institute and community population of 20,000 people voluntarily accepted cerebrovascular hemodynamic test of the physical examination crowd to understand the high risk of stroke population baseline study.From the five dimensions of basic information,physical measurement,laboratory tests,risk factors of stroke and baseline detection of cerebrovascular function,we can get a comprehensive understanding of the basic status quo of stroke at risk in Urumqi.3.This study aimed at 2000 cases of high-risk individuals file management intervention for more than 1 year to explore the intervention group and control group of cerebrovascular function indicators,biochemical indicators,the effective control of risk factors and lifestyle changes and other indicators.Results: 1.Fifteen articles involving 1760 cases were included in this study.The results showed that compared with the control group using aspirin treatment,the pooled RR(95%CI)of the effectiveness rate and the cure rate in the group using Ginkgo combined with aspirin in the treatment of cerebral infarction were 1.16[1.12,1.20] and 1.51[1.31,1.75],respectively.The pooled SMD(95%CI)of NILSS,blood viscosity,plasma viscosity,high shear viscosity of blood,and fibrinogen were-1.65[-1.99,-1.31],-3.31[-5.03,-1.6],-3.20[-3.88,-2.51],-7.54[-8.11,-6.97],and-3.37[-4.86,-1.88],respectively.All of the differences were statistically significant(P<0.05).2.The detection rate of high-risk stroke patients in Urumqi is related to nationality and age.The detection rate is high.Absence of exercise and dyslipidemia are the main risk factors for stroke in all age groups,and the differences are statistically significant.Stroke differs The results of laboratory tests of risk populations showed significant differences in uric acid,fasting blood glucose,HbA1 c,triglyceride,LDL,homocysteine,and serum C-reactive protein levels.In terms of unhealthy lifestyle,the lack of exercise,smoking,and regular heavy drinking are more obvious,and the differences are statistically significant.There are statistically significant differences in the indicators of pulse wave velocity,minimum blood flow velocity,and peripheral resistance in cerebrovascular hemodynamic parameters of high-risk stroke patients.Logistic regression analysis showed that Hypertension,previous TIA,TC,past history of stroke,overweight or obesity,diabetes,homocysteine,lack of exercise,smoking,HDL-C are important influencing factors of cerebral hemodynamic abnormalities,and the OR value is 2.728,2.665,2.382,2.344,2.085,1.678,1.501,1.520,1.292,0.562,All indicators have statistical significance.3.The intervention group was effective in identifying risk factors for stroke in high-risk stroke populations,stroke-related disease knowledge,eating habits,exercise habits,smoking,alcohol consumption,psychological,risk factor control levels,cerebral hemodynamic pulse wave velocity(L,R),theminimum flow rate(L,R),peripheral resistance(L,R),brain function scores and other indicators were statistically significant.People over the age of 40 are the focus groups for the prevention and treatment of cerebrovascular diseases.High-risk groups after risk assessment for stroke should take intervention measures in time,which can effectively reduce the occurrence and development of stroke.Hypertension,diabetes mellitus,dyslipidemia,overweight or obesity,smoking,alcohol consumption,lack of exercise,cerebral hemodynamic abnormalities,not taking medication on time,and major accidental disturbances are all significant risk factors in univariate analysis.Conclusion: 1.Compared with aspirin therapy,Ginkgo injection combined with aspirin achieved a better curative effect for successful treatment of cerebral infarction.However,in view of the limitations of this study,it is suggested that large-scale,high-quality studies should be performed to further verify the above conclusion by critical outcome indicators.The auxiliary lifestyle intervention of Ginkgo biloba health products is ideal for primary prevention.2.The prevalence of risk factors such as hypertension,dyslipidemia,diabetes,smoking history,and lack of exercise among men and women was higher than that of women.Overweight/obesity,lack of exercise,smoking,hypertension,and hyperlipidemia were the main risk factors for stroke.Obesity,lack of exercise and smoking,and hypertension were the top four risk factors for the detection rate of high-risk groups.Smoking,regular heavy drinking,and lack of exercise are the main unhealthy lifestyles in high-risk stroke groups in Urumqi.The ROC curve method was used to evaluate the predictive model of hypertension,diabetes,overweight or obesity,lack of exercise,history of previous stroke,history of transient cerebral ischemia,smoking history,drinking history,etc.The area under the ROC curve was 0.783,which was significantly different from the area under the opportunity line.This shows that the model has good prediction effect and can accurately predict the risk of stroke in Urumqi.3.The comprehensive intervention can effectively identify the risk factors of stroke,knowledge about stroke-related diseases,diet,exercise habits,smoking,alcohol,psychology,control of risk factors and cerebral hemodynamics.The comprehensive intervention can reduce the risk of stroke.The effect of risk is clearly better than conventional management.
