osteoporosis patients about 90 million people.Osteoporosis does tremendous harm,the disability and mortality of osteoporotic fracture is incresing,whitch not only affect the quality life of patients seriously,but also bings complicated nursing,medical problems,which need to Put in huge manpower and material resources,high cost,and cause burden of family,society and economy.In recent years,as people pay more attention to osteoporosis,more and more countries and regions do surveys about the prevalence of osteoporosis.China has several provinces and cities have osteoporosis Prevalence survey.But these investigations on osteoporosis prevalence and related factors analysis is given Priority to,the lack of the public understanding screening,treatment of osteoporosis situation,the analysis of the situation and influence factors,and these Problems are also very worthy to study.As the most populous province of henan Province,there is no mass for epidemiological investigation of henan people osteoporosis,only about henan nanyang and zhengzhou small sample of the regional survey [2,3],and there is no large size sample of a crowd of systemic understanding,screening and treatment of osteoporosis.Because of the high rates and great harm of osteoporosis,except for knowing the prevalence and influence factors of osteoporosis,it is of great significance to the prevention and treatment of osteoporosis and prevent osteoporosis fracture,improve the quality of life through the positive education and intervention,to improve the recognition of the masses,screening and treatment.Objectives By investigating the patients,screening rate,treatment rate and awareness rate of osteoporosis in the first affiliated hospital of zhengzhou university hospital,we discuss the relevant factors of which preliminary understanding,awareness screening,treatment,and present situation of osteoporosis and its influencing factors in henan province.And we take a positive education and intervention to improve the masses awareness,screening and treatment of the disease,take active prevention and treatment of osteoporosis and prevent osteoporosis fracture,improve patient quality of life,save medical expenses.Methods For 6902 hospitalized patients from 1st,July,2016 to 7th,July,2016(not including those under 18,mental abnormalities,unconsciousness,unable to correct the objective to complete the questionnaire and refused to attend the investigators)to investigate.We adopted questionnaire,physical examination and laboratory examination method to understand the crowd for osteoporosis,screening rate,treatment and influence factors analysis.We compared awareness rate,screening rate and treatment of osteoporosis between the different groups using chi-square test.We analysed the influence factors of awareness rate,screening rate in osteoporosis between the different groups using Logistic regression.(P < 0.05)difference was statistically significant.Results 1.The survey participants included 6902 hospitalized patients as the research object.The total awareness rate of osteoporosis was 50.9%.High-risk population of osteoporosis is 50.1%(positive for a minute test).Screening rate in a high-risk population of osteoporosis was 8.23%.The prevalence of osteoporosis was 45.3% in those people who take examination about bone mineral density.The treatment rate of osteoporosis patients was 71.32%.2.Multiple factors analysis results show that the possibility of knowing osteoporosis is 1.525 times that of the rural population(OR = 1.525,95% CI: 1.525,1.715).The possibility of awareness for osteoporosis in people who are primary school or flowing cultural is lower than people who are junior college and above(OR=0.373,95%CI:0.314,0.444).The possibility of understanding osteoporosis in people who are Positive in one minute test was 1.264 times of the negative(OR = 1.264,95% CI: 1.264,1.394).The possibility of understanding osteoporosis in people who suffer from high blood pressure is 1.251 times than that without(OR=1.251,95%CI:1.108,1.412).The possibility of understanding osteoporosis in people who suffer from diabetes is 1.217 times than that without(OR = 1.217,95% CI: 1.217,1.359).The possibility of understanding osteoporosis in people who are dyslipidemia is 1.207 times than that without(OR = 1.207,95% CI: 1.207,1.337).3.In a high-risk population of osteoporosis,multi-factor analysis showed that,with increasing age osteoporosis screening rate gradually increased,the possibilities of osteoporosis screening in adults over 80 is 1.771 times than people aged 18 to 39(OR = 1.771,95% CI: 1.771,1.755).The possibilities of osteoporosis screening in the cities and towns is higher than rural(OR = 1.965,95% CI: 1.965,2.686).The possibility of osteoporosis screening in Junior college OR above degree of culture is higher than under primary and culture(OR = 1.081,95% CI: 1.081,1.125).Knowledge workers(OR = 2.709,95% CI: 1.455,5.045)and unemployed(OR = 1.895,95% CI: 1.895,1.895),the possibility of osteoporosis screening was higher than that of manual workers.The possibility of osteo Porosis screening in people Combined with hypertension is 1.342 times than normal(OR = 1.342,95% CI: 1.342,1.798).The possibility of osteoporosis screening in people who are dyslipidemia is 3.925 times than normal(OR = 3.925,95% CI: 3.925,5.5729).4.The results single factor and multiple factors analysis showed that only Professional factors of osteoporosis treatment.Knowledge workers(OR = 3.322,95% CI: 1.250,8.829)and the unemployed personnel(OR = 4.093,95% CI: 4.093,11.990)the possibility of osteoporosis treatment were higher than manual workers.Conclusion 1.Compared with other surveys,the rate of osteoporosis awareness is higher,the screening rate of osteoporosis in high-risk population is lower,the treatment rate of osteoporosis is higher in the survey.2.The people is easier to know osteoporosis who are in cities or owns,higher cultural level,test Positive for a minute,hypertension,diabetes,dyslipidemia.3.The people is easier to screen osteoporosis who are older,urban and cultural level higher,the knowledge workers and unemployed,high blood pressure,dyslipidemia.4.Knowledge workers and unemployed Personnel is more likely to have osteoporosis therapy than manual workers.Part II Diabetes mellitus Awareness,Treatment,Control and Influence FactorsBackground Diabetes has become an important public health problem in China.Epidemiological survey in 2010 showed that the total prevalence rate of diabetes in China has reached 11.6%.Diabetes have great harm,the present study has confirmed that long-term hypertension stimulation can cause the eyes,kidneys,and complications and cardiovascular disease,seriously affect the quality of patients life,and even life-threatening,at the same time raised the medical expenses.In addition,recent research reports suggest that diabetes is a risk factor of tumor.In recent years,along with the rising prevalence of obesity,diabetes,more and more attach importance to diabetes,more and more countries and regions conducted surveys about diabetes epidemiology..Results show that the prevalence is higher,but the awareness,treatment and the success rate of patients is low.And studies show improved the situations of glycemic control is the key to prevent diabetes complications.China has the world's largest population of diabetes,and as the largest population of China,henan province,the diabetes knowledge,the condition of control and treatment in type 2 diabetic patients may also be worrying.So we are very necessary to understand the situations of know,treatment and control situation and related factors in the diabetes population of henan province,and take positive intervention,to improve the control rate in diabetes and reduce complications,that have great significance.Objectives Investigate the awareness rate,treatment rate,and the controling status of diabetic patients in first affiliated hospital of zhengzhou university hospital,explore the related factors,So as to get a brief understanding of the satus of awearness,treatment,control and influencing factors in diabetes.Positive intervention,and raise the level of knowledge of diabetes,treatment and control,prevent and delay the onset of diabetes complications,further save medical expenses.Methods Those in the study are 2001 of 6902 diabetic patients.We adopted questionnaire,physical examination and laboratory examination method..Understand the rate of awareness,treatment,reach the standard in those people,and analyzed the influencing factors.Compared the awareness rate,treatment rate and controlling rate in different groups using significant.Results 1.This study surveyed 6902 people,2001 people with diabetes,the prevalence of diabetes was 29.0% in this study.Awareness rate of diabetes in patients with diabetes was 49.5%,the treatment rate was 33.3%,the control rate in diabetes who accepted treatment is 47.9%.2.Multiple factors analysis shows that women are less likely to know their diabetes(OR = 0.734,95% CI: 0.734,0.960).Compared with patients who are 18 to 39 years old,the awareness rates of the remaining 5 age groups are higher,including the possibility of awareness diabetes in 60-69 age group was 7.499 times than groups 18 to 39 years old group(OR = 7.499,95% CI: 7.499,16.877).The possibility of awareness diabetes in people who take regular exercise is 3.159 times than never exercise(OR = 3.159,95% CI: 3.159,4.745),the possibility of awareness diabetes in people who take occasionally exercise is 5.551 times than never exercise(OR = 5.551,95% CI: 5.551,7.684).The awareness rate in people who eat more salt(OR = 0.822,95% CI: 0.705,0.401)and who eat little salt(OR = 0.277,95% CI: 0.277,0.396)is lower tha people who eat moderate salt.The possibility of a minute test positive for diabetes awareness is 1.444 times than the negative(OR = 1.444,95% CI: 1.444,1.887).The possibility of diabetes awareness in people with hypertension is 95.967 times than without((OR = 95.967,95% CI: 95.967,191.489),The possibility of diabetes awareness in people with dyslipidemia is 1.952 times of those without(OR = 1.952,95% CI: 1.952,2.694).3.In the diabetic patients who accepted treatment,multi-factor analysis showed that,Compared with patients who are 18 to 39 years old,the diabetes awareness rate is higher than the remaining 5 age groups,including 60-69 age group the highest possibility of diabetes,6.845 times for 18 to 39 years old age group(OR = 6.845,95% CI: 6.845,14.985).The possibility of diabetes treatment in people who take regular exercise is 6.913 times than never(OR = 6.913,95% CI: 6.913,10.309),the possibility of diabetes treatment in people who take occasional exercise is 29.458 times than never(OR = 29.458,95% CI: 29.458,42.189).the possibility of diabetes treatment in people who eat more salt and who eat little salt is lower tha people who eat moderate salt.The possibility of diabetes treatment in people who are positive in a minute test is 1.538 times than the negative(OR = 1.538,95% CI: 1.538,2.009).The possibility of diabetes treatment in diabetic patients who suffered from hypertension disease is 8.073 times than without(OR = 8.073,95% CI: 8.073,11.436).The possibility of diabetes treatment in diabetic patients with dyslipidemia is 1.714 times than not(OR = 1.714,95% CI: 1.714,2.403).4.The blood sugar controlling rate of diabetic patients,multi-factor analysis results show that the possibility of blood sugar controlling success in single patients is lower than married people(OR = 0.704,95% CI: 0.704,0.330).The possibility of blood sugar controlling success in people who take occasional exercise is 1.664 times than never(OR = 1.664,95% CI: 1.664,2.109).The possibility of blood sugar controlling success in people who take non-vegetarian food is lower than a hun vegetable(OR=0.152,95%CI:0.0792,0.294).The possibility of blood sugar controlling success in people who have diabetic family history is 0.277 times than not(OR=0.277,95%CI:0.200,0.385).Conclusion 1.In this study,the awareness rate,treatment rate,and controling rate for diabetes is higher than in the 2010 national survey,but is still low compared with developed countries.The awareness rate,treatment rate is lower compared with other provinces and cities in our country,but the controling rate is higher.2.The people who are male,older,physical exercise,vegetarian food,have moderate salt,positive in a minute test,merging high blood pressure or dyslipidemia are easier to know diabetes.3.The diabetic patients who are male,older,physical exercise,vegetarian food,have moderate salt,positive in a minute test,merging high blood pressure or dyslipidemia are likely to accept treatment.4.The possibility of diabetes control success in people who are married people,physical exercise,balanced diet is higher,but the possibility in people who have diabetic family history is lower.Part III Dyslipidemia Awareness,Treatment,Control and Influence FactorsBackground With the development of the economy,,the improvement of people's living standard and exetension of life,chronic non infectious diseases has been one of the reason of disorder people ‘s health,increasing mortality,and medical expenditure.dyslipidemia as one of the most common non infectious diseases,does great harm to the society.It can lead to the AS,so that leads to cardiovascular disease.Now in our country,,the death of cardiovascular disease accounts for 41% in all death reasons.,that is to say,the death that cardiovascular disease caused has been the first reason that lead to residents death.Even though there are many surveys about abnormal disease,but little about awareness,cure rate and control condition,many of them manage in its damage and prevalence.A little cities try to do relavant study.As for HENAN province,a populous province,still has no epidemiological investigation about dyslipidemia.So as investigation about awareness,cure rate,control rate in the masses.The prevalence of dyslipidemia is high,does great harm,awareness,cure rate,control rate are high,it's necessary for us to come to know the awareness,cure rate,control condtion in our province,do active universal education,improve broad masses knowledge of disease,cure and control,prevent dyslipidemia and cardiovascular disease that it leads to,improve the quality of life,save medical expenditure.Objective DO Investigation of Zhengzhou university First Affiliated Hospital inpatient about awareness,cure rate,control rate of dyslipidemia and discuss the relavant factors so as to understand the HENAN masses about awareness,cure rate,control rate of dyslipidemia and discuss the relavant factors.Do active intervention and improve the consciousness of awareness,cure rate,control rate of dyslipidemia.Prevention harm and save medical expenditure.Methods There are 6902 people in the study,make 4186 people as study object.Use questionnaire investigation,physical examination,laboratory examination to know the awareness,cure rate,control rate and influence factor.Use Chi square test to compare different groups about awareness,cure rate,control rate,use Logistic regression method to compare different groups about awareness,cure rate,control factors.P < 0.05 has statistical significance.Results 1.6902 people have been investigated in the study,4186 persons present abnormal blood lipids,that is to say,the prevalence of abnormal blood lipids is 60.6% in the research.The awareness is 25.0%,the cure rate is 17.4%,the control rate is 52.3% in abnormal blood lipids groups.2.In the aspect of abnormal blood lipids,multivariate analyses shows.urban dwellers are more easily to know that they have been suffer from abnormal blood lipids.Compare with 18-39 age group,70-79 age group is easier to know abnormal blood lipids.Mental workers are more easily to know abnormal blood lipids than manual workers.People with animal food diet is easier to know dyslipidemia than people with vegetarian diet.People with medium diet is also easiler to know dyslipidemia than those people with salty dietary and light dietary,the possibilty of knowledge of dyslipidemia in those people with DM is higher than people without DM.People with dyslipidemia family history is easiler to know dyslipidemia than those without family history.3.In cure group of dyslipidemia,multivariate analyses shows,compare with 18-39 age group,the possibility of accepting to adjust lipids in 70-79 age group and more than 80 age group is higher.The possibility of accepting adjust dyslipidemia in mental workers is bigger than manual workers.The possibility of those who exercised more and those who exercised now and then are all lower than those who never exercised.The possibility of accepting adjust dyslipidemia in salty dietary andlight diet are all lower than those with medium dietary.The possibility of adjusting lipids in group with DM is 12.154 times than group without DM,the possibility of adjusting lipids in group with dyslipidemia family history is 6.484 times than those without family history.4,In aspect of control rate of dyslipidemia,multivariate analyses shows,compared with18-39 age group,the possibility of controling dyslipidemia is higher inthe else 5 age groups,the 6-69 age group is the highest of all.The possibility of accepting adjust dyslipidemia in mental workers is bigger than manual workers.Compared with people who exercised more,the possibility is lower in those who exercised now and then and never exercise.Compared with those people without DM,the possibility of control dyslipidemia is lower in people with DM.Conclusion 1.in the study,the prevalence of dyslipidemia is higher than the whole country and other provinces or cities.awareness,cure rate and control rate are all higher than the investigation in china on2002 and 2010 and in JILIN province.,but still lower than the study in the USA and other developed country.but compared with other cities in china,awareness,cure rate are lower,but control rate is higher.2.factors such as age increasing,mebtal workers,animal food diet,with DM,and with dyslipidemia family history will make bigger impossibility of have a knowledge for dyslipidemia.3.factors such as age increasing,mental workers,people who never do exercise,medium diet,animal food and vegetarian diet,with DM,and with dyslipidemia family history will make bigger impossibility to accept to adjust the dyslipidemia.4.factors such as age increasing,mental workers,people who often do exercise,medium diet will make higher possibility to control the lipids up to the standard.the possibility of control rate is low for people with D M. |
