Objectives:To investigate the effect and function mechanism of Rheum Offcinale Baill on congestive heart failure.Methods:In this study,we try to investigate the effect and function mechanism of Rheum Offcinale Baill on congestive heart failure through two points of view which were clinic and animals experiments.It can be divided into four sections:(1)Clinical research for the mechanism that Rheum Offcinale Baill could treat congestive heart failure.In this study congestive heart failure patients treated in the Second Affiliated Hospital of Zhengzhou University were divided into control group(treated with ACEI,ARB,beta-blockers et al)and treated group(on the basis of the basic treatment,with Rheum Offcinale Baill at 10:00 AM daily).the daily intake of Rheum Offcinale Baill were 10 g,14 days later,measure LVEF?LVSV and LVEDD,the content of BNP,AngII,ALD,Renin,IL-6 and THF-? respectively;(2)investigate the effect and function of Rheum Offcinale Baill on cardiac function and BNP level of rats.Mice were injected with azithromycin to establish the congestive heart failure model.Rats were randomly divided into blank control group,model control group,high dose(200 mg/Kg)group,middle dose(100 mg/Kg)group and low dose(50 mg/Kg)group,blank control group were injected and intragastric gavaged physiological saline 0.3 ml;model control group were injected azithromycin10 mg/kg,intragastric gavaged physiological saline 0.3 ml,three dose group were intragastric gavaged Rheum Offcinale Baill 0.3 ml and injected azithromycin10 mg/kg at the same time for 21 days.The LVEDD,LVESD and LVEF were measured by echocardiography;the level of BNP that in plasma were measured through ELISA method;the level of Ang? that in plasma and myocardial were measured through RIA method.(3)investigate the effect and function of Rheum Offcinale Baill on the levels of oxidative stress and inflammatory cytokines of model animals.Mice were injected with azithromycin to establish the congestive heart failure model.Rats were randomly divided into blank control group,model control group,high dose(200mg/Kg)group,middle dose(100 mg/Kg)group and low dose(50mg/Kg)group,blank control group wre injected and intragastric gavaged physiological saline 0.3 ml;mice in model control group were injected azithromycin10 mg/kg,intragastric gavaged physiological saline 0.3 ml,three dose groups were intragastric gavaged Rheum Offcinale Baill 0.3 ml and injected azithromycin10 mg/kg at the same time for 21 days.The content of SOD that in myocardial were measured through Western blot method;the content of SOD?MDA?TNF-?,IL-6 and CRP that in plasma were measured through ELISA method;the content of NF-kB were measured by EMSA;(4)investigate the effect and function of Rheum Offcinale Baill on myocytes apoptosis of model animals.Congestive heart failure mice model were established through abdominal aortic method.Rats were randomly divided into model control group,sham group(no treatment),high dose(200mg/Kg)group,middle dose(100 mg/Kg)group and low dose(50 mg/Kg)group,20 rats in eaeh group,Rheum Offcinale Baill intragastric gavage started at 4 weeks after surgery,one time everyday lasted for 8 weeks.Model control group were intragastric gavaged same volume physiological saline.The myocytes apoptosis were detected through TUNEL method;the content of TNF-?,IL-6 that in myocardial were measured through ELISA method;the content of NF-kB were measured by EMSA;the express level of Bcl-2?Bax were detected through Western blot method;the express level of mRNA of Bcl-2?Bax and Caspase-3 were detected through RT-PCR method.Results:(1)After treated with Rheum Offcinale Baill,the LVEF,LVSV of congestive heart failure patient were improved,LVEED,the content of BNP,Ang?,ALD,TNF-?,IL-6 and activity of PRA were declined,cardiac function were improved.(2)The LVEDD and LVESD,content of BNP and AngII of model control group were improved compared with blank control group,and LVEF decreased.After treated with Rheum Offcinale Baill,the LVEDD and LVESD,content of BNP and AngII of model control group were decreased,LVEF were improved obviously as dose of Rheum Offcinale Baill increased;(3)The contents of MDA,TNF-?,L-6,CRP,activity of NF-?B of model control group were improved compared with blank control group and then decreased after treated with Rheum Offcinale Baill,the content changes of SOD were contrary;(4)After treated with Rheum Offcinale Baill,the level of Bcl-2 and its mRNA about model animals wre raised,the activity of NF-?B and level of Bax?Caspase-3 and its mRNA wre decreased,rate of myocytes apoptosis were also dropped.Conclusions:(1)Rheum Offcinale Baill has good effect on congestive heart failure,the function mechanism maybe related to Rheum Offcinale Baill could inhibit the levels of inflammatory cytokines,reverse the cardiac remodeling and inhibit the constantly activite of RAAS;(2)Rheum Offcinale Baill could inhibit ventricular enlargement and improve left ventricular systolic function,it maybe related to Rheum Offcinale Baill could inhibit the activation of RAAS;(3)Rheum Offcinale Baill shows protection effects on congestive heart failure maybe by inhibiting the activation of NF-kB,through inhibited levels of oxidative stress,eliminated inflammatory cytokines through the way of diarrhea,anti-infective,then reduced the expess of inflammatory cytokines;(4)Rheum Offcinale Baill could restrain the occurrence and/or development of myocytes apoptosis,reverse the cardiac remodeling and improve heart function.
