ObjectiveBy studying the lymphocyte subsets,NK cells(natural killer cells),NK cell function and T helper cell subsets in chronic insomnia patient with yang deficiency constitution,this research try to analyse the immune disfuntion in chronic insomnia patient with yang deficiency constitution from the perspective of natural killer cell and T cell.This study will provide theoretical basis for micro-differentiation of chronic insomnia patient with yang-deficiency constitution.MethodsThis study is a cross-sectional study in patients with chronic insomnia and people with normal sleep.Chronic insomnia group will be divided into two subgroup:yang deficiency group and non-yang deficiency group.The non-yang deficiency group will be futher divided into biased constitution group and gentle constitution group.General information collection,Chinese medicine constition survey questionnaire,and Pittsburgh sleep scale was assessed before the test.Venous blood was collected and tested for lymphocyte subsets,nk cell subsets and its expression of granzyme B and perforin,nk cells function,Thl,Th2,Th17 subsets.Data will be analysed within chronic insomnia group and normal sleep group.Subgroup analysis will be applied in chronic insomnia group.Correlation analysis will be conducted between the degree of yang deficiency and the index mentioned above.Results1.The lymphocyte subsets:Chronic insomnia group(n = 59)of had lower lymphocytes number,NK cells number,T lymphocytes number,inhibitory/cytotoxic T cells number,B cells number than normal control group(n = 30).In the study of two subgroups,yang deficiency group had lower nk cells number and percentage than non-yang deficiency group,but had higher T cells number and percentage,Th cells number than non-yang deficiency group.In the study of three subgroups,yang deficiency group had lower NK cells percentage than gentle constitution group.In chronic insomnia group,the total number of T lymphocytes(r=0.273),total T lymphocyte percentage(r=0.37)and were positively correlated with degree of Yang deficiency,the percentage of NK cells were negatively related with Yang deficiency degree(r= 0.273).2.NK cells subsets and its expression of granzyme B and perforinCD56+CD16"NK cells:chronic insomnia group(n= 32)had higher CD56+CD16-NK cell percentage than normal control group(n=28).In the study of two subgroups,yang deficiency group had lower CD56+CD16-NK cells granzyme B percentage,granzyme B fluorescence intensity and perforin percentage than non-yang deficiency group.In the study of three subgroups,yang deficiency group had lower CD56+CD16-NK cells granzyme B percentage than gentle constitution group.Yang deficiency group had lower granzyme B fluorescence intensity than non-yang deficiency biased constitution group and gentle constitution group.CD56+CD16+NK cells:Chronic insomnia group(n = 32)had lower CD56+CD16+NK cells percentage than normal control group(n = 28),but its perforin percentage and perforin fluorescence intensity were higher than normal control group.In the study of two subgroups,yang deficiency group had lower CD56+CD16+NK cells percentage,granzyme B percentage,perforin percentage and perforin fluorescence intensity than non-yang deficiency group.In the study of three subgroups,yang deficiency group had lower CD56+CD16+NK cells granzyme B percentage than non-yang deficiency biased constitution group and gentle constitution group.Yang deficiency group had lower CD56+CD16+NK cells perforin fluorescence intensity than gentle constitution group.CD56+CD16+NK cells:Chronic insomnia group(n = 32)had lower granzyme B percentage than normal control group(n = 28),but it had higher granzyme B and perforin fluorescence intensity than normal control group.In the study of two subgroups,yang deficiency group had lower granzyme B percentage,perforin percentage and perforin fluorescence intensity than non-yang deficiency group.In the study of three subgroups,yang deficiency group had lower granzyme B percentage and perforin percentage than non-yang deficiency biased constitution.Yang deficiency group had lower perforin fluorescence intensity than gentle constitution group.In chronic insomnia group,the CD56+CD16+NK cells percentage(r=-0.446),CD56+CD16+NK cells granzyme B percentage(r=-0.647),CD56+CD16+NK cells perforin fluorescence intensity(r=-0.465)and CD56+CD16-NK cells granzyme B fluorescence intensity were negatively related with yang deficiency degree.3.NK cells activity and secretion functionNk cells activity and secretion function had no difference between Chronic insomnia group(n=8)and normal control group(n=7).4.Thl,th2,th17 subsets:Chronic insomnia group(n=58)had lower Th1/Th2 ratio than normal control group(n=22).In the study of two subgroups,yang deficiency group had lower Thl cells percentage,Th2 cells percentage and Th17 cells percentage than non-yang deficiency group.In the study of three subgroups,yang deficiency group had lower Thl cells percentage and Th2 cells percentage than non-yang deficiency biased constitution group.Yang deficiency group had lower Th17 cells percentage than non-yang deficiency biased constitution group and gentle constitution group.In chronic insomnia group,Thl cells percentage(r=0.452),Th2 cells percentage(r=0.39),Th17 cells percentage(r =0.387)were negatively related with yang deficiency degree.ConclusionThe immune dysfunction of chronic insomnia is characterized as the decline of NK cells and T cells numbers and the shift of their subgroups.Chronic insomnia with yang deficiency constitution is a phenotype that its immune function prone to be influenced by insomnia.Chronic insomnia with yang deficiency constitution is characterized by the decline of immune activation which is negatively related with yang deficiency degree.
