Osteoporosis is a typical chronic degenerative disease that has become one of the five major geriatric diseases(hypertension,cardiovascular,hazeia,cancer and osteoporosis).China is one of the most populous countries in the world,and the aging process is rapidly increasing.With the rapid increase in osteoporosis,it will bring great challenges to national and social medical resources.Old people once suffering from osteoporosis,the treatment will be very difficult because of the reduction of gonadotropin which influences the osteoblast precursor cell differentiation,increasing of osteoclast activity,and the internal and external environment of bone being not easy to change.Although some of the current drugs can promote osteoblast osteogenesis or inhibit osteoclast activity to achieve the purpose of intervention in the process of osteoporosis,but the long-term safety of drug intervention is uncertain,and now some of the drugs have been proved to have increased the risk of cardiovascular and other diseases.Therefore,the US Food and Drug Administration(FDA)recommended that osteoporosis drug intervention should be very cautious.The best strategy for treatment of osteoporosis is prevention.To prevent osteoporosis by increasing the peak bone mass obtained in early adulthood and reducing the rate of bone loss with aging.In the regulation of human bone,some scholars have proposed the theory of Mechanostat.The Mechanostat theory was used to analyze the changes in the stress environment.However,there are still many controversies about the effects of different stress on the bone,such as the weight of the body,the fat-free mass and the fat mass,and the effects of different sports on the bone.Therefore,it is very important to construct the method of osteoporosis prevention and treatment based on the Mechanostat theory.This study was focus on on the following aspects:1.Study on the effect of static load on boneIn view of the differences in body weight,FFM5 FM and bone quality in different races,gender,age groups,finding the relationship between body composition and bone quality will help to develop scientific intervention guidance method.In our studies,a total of 441 middle-aged men and women and 568 elderly men and women were enrolled.The body composition data and bone quality of the subjects were obtained.The relationship between body composition parameters and bone quality of different age groups and different sexes was analyzed.The osteoporosis ratio of older women was higher than that of older men(P<0.05).In addition,Fat free mass had significant positive correlation with bone quality with men and women(R =0.271-0.406,P<0.001).There was a gender difference in the relationship between FM and bone quality.There was a weak correlation between FM and female bone quality,which it was not found in men.2.Study on the relationship between exercise capability and bone qualityIn order to study the relationship between exercise ability and bone mass,this study analyzed the relationship between the lower limb muscle volume,the grip strength and the calcaneal bone quality in 937 young people aged 20-40.The results showed that the body's muscle mass and muscle strength had positive correlation bone quality,sspecially in men,and this relationship is site-specific(P<0.05).These data confirmed that increasing the muscle mass and muscle strength of a particular site would contribute to the promotion of bone quality at that site.In addition,this study analyzed the relationship between cardiopulmonary endurance,vital capacity and bone quality in 945 men aged 20-40 years.The results showed that the cardiopulmonary function was significantly positively correlated with bone quality(P<0.05).Higher level of cardiopulmonary function contributed to the promotion of bone quality.These above studies provided data for the development of bone health promotion strategies.3.Study on the bone health promotion program reasoning scale and personalized bone health promotion service systemBased on the theory of Mechanostat,this study established the bone stress regulation model,and proposed the bone health promotion program reasoning scale,and this study developmented a bone health promotion service system basing on the individual physical function data.Through the analysis of individual bone status and the risk factors of bone quality and according to the individual's physical function,the system developed to reason out the targeted of bone health promotion program.Then the relevant exercise prescription can be pushed to the individual mobile terminal,the system will monitor the user movement.The results of the process will be transmitted to the big data center of national health.Through the dynamic monitoring of individual bone changes and changes in bone risk factors,this system was in order to achieve the promotion of bone health closed-loop intervention management system.In addition,this study designed an intelligent bone health promotion intervention device,this device can perform the bone stimulation program from bone health promotion service system,and this device can exercise for all parts of the body to improve the muscle,muscle strength and muscle mass,so that the bone quality can be promoted.4.A Study on the population follow-up study on the effect of exercise on bone healthIn order to study the practical effect of exercise intervention of the promotion of bone health,this study randomly selected the middle-aged people who were prone to osteoporosis in the general population,and finally selected 81 middle-aged or elderly people.After 6 months aerobic run and low-intensity equipment exercise,the results showed that exercise intervention had a significant effect on the bone quality in both elder men and women(P<0.05).Meanwhile,106 drug-dependent women were selected for bone evaluation and intervention studies.First,the differences of bone quality between drug-dependent women and healthy women were studied,and the influencing factors of bone quality between the study group and the control group were analyzed.We found that long-term drug dependence would lead to bone loss.Then,the drug-dependent women were divided into low-bone group and normal bone group.Their bone quality and body composition were measured,and the exercise and diet were investigated.After 7 months aerobics,the bone quality of low-bone group had significantly increased(P<0.05).but there was no significant improvement in bone quality of the normal bone group(P>0.05).The study on the exercise intervention of bone quality of drug-dependent women was seldom discuss.The results confirmed that exercise intervention can improve the bone health of drug-dependent women with low bone quality,but differentiated intervention program should taken on to different groups with different bone conditions.
