| Objective Chen Xiaoxia is an instructor of the fifth Project of academic experience inheriting of the old famous Chinese medicine experts.She has been engaged in clinical working,scientific research on kidney diseases for more than 30 years and achieved a lot.She has formed innovative her style of Chinese medicine diagnosis and treatment and had the profound attainments and the academic ideas on the kidney. In order to explore the experience of tutor in the treatment of membranous nephropathy, we tried to summarize the academic experience in the treatment of chronic kidney, and carried out clinical validation.Method Through theoretical study and clinical teacher’s method to sum up the academic experience of tutor in the treatment of chronic kidney disease; Through in-depth exchanges between teachers and students, the method of medical record analysis to discuss the experience of tutor in the treatment of membranous nephropathy; A randomized controlled study was used to validate the experience of the tutor in the treatment of membranous nephropathy.ResultIn the first part, the author introduces the Chen Xiaoxia’s experience of treatment of chronic kidney disease.1.Pay great attention to status of deficiency of Spleen qi on pathogenesis of chronic kidney disease. By the treatment, we shall give more consideration on stomach and spleen especially,don’t forget the Spleen Yin.2. Attaches great importance to the theory of Fengshi. She deemed the kidney was impacted by "Fengshi"and wasn’t incharg of proteinuria. So we can detect a large amount of proteinuria.We believe "Fengshi" played a very important role on chronic kidney diseases. "Fengshi" is always a part of the pathogenesis of chronic kidney disease. 3.Take the analogy like Main and collateral channels, renal capillaries is hardening. By the treatment,we invigorate the circulation of blood.4. Professor Chen Xiaoxia maked the best use of group of traditional chinese medicines,and enhanced the effect of medicine.5. In the late disease, Professor Chen Xiaoxia maked the best use of paste. She pointed out,we need riched Spleen and kidney,got rid of warm and dry.The second part is summary of professor Chen Xiaoxia’s experience of treatment of membranous nephropathy, the author summarized as follows:Deficiency of Spleen and Kidney qi is the base of pathogenesis of membranous nephropathy. blood stasis is the secondary. "Fengshi" is always a part of the pathogenesis of chronic kidney disease. Deficiency of Spleen and Kidney qi is the most importance pathogenesis。we thought:"Yin-forming" leads to the Glomerular basement membrane thickened. Blood stasis block is impotant in the membranous nephropathy. "Fengshi"is the initiating factors at membranous nephropathy.Professor Chen Xiaoxia develops therapeutic principles,tries to regulate and nourish the deficience of spleen and kidney qi, invigorate the circulation of collaterals,remove the "fengshi".She inductives the membranous nephropathy to three card types.They are referred to as "deficience of spleen and kidney qi","blood stasis block", "fengshi"。 The three types’relationship is parallel.Based on this theory,we use azathioprine tablets and group of traditional chinese medicines which is conducive to the membranous nephropathy.We emphasize to body untouched by evil.In the third part,we carry out clinical validation.We has declared subject and for clinical randomized controlled trials, With prednisolone+CTX(P+CTX) classic scheme. The result shows that:azathioprine tablets+Chinese medicine group (TW+ Chinese traditional medicine group) can effectively reduce albuminuria, well tolerated. Compared with the control group (P+CTX), experimental group relief time earlier,3 months start proteinuria reduction, most patients appear to ease is 6 months after drug use; and most of the patients to alleviate P+CTX group in 12 months time. Experimental group(TW+Chinese traditional medicine group) plasma albumin levels rise since 6 months and 12 months increased obviously. P+CTX group plasma albumin levels rise since 3 months and 6 months increased obviously. Experimental group(TW+ Chinese traditional medicine group) of plasma albumin levels rise slowly. Two groups have liver function damage and the upper respiratory tract infections, pneumonia cases, after anti-infection treatment all can continue to complete clinical test and verify.1 patient in the control group(P+CTX group) complicated with severe pneumonia. But TW+Chinese traditional medicine group, fewer complications, side effect is small, the patient easy to tolerance, have very good clinical curative effect. |
