Purpose To observe the effect of intravitreal injection of Ranibizumab for exudative age-related macular degeneration(eAMD)with serous pigment epithelial detachment(sPED).Method This study reviewed twenty-three wet age-related macular degeneration combined serous pigment epithelial detachment cases that employed the intravitreal injection of ranibizumab at The Tenth People's Hospital affiliated to Shanghai Tongji University from June 2011 to June 2014.The 23 patients received the intravitreal ranibizumab injection of 3 loading doses monthly plus pro re nata,every patient have regular monthly follow-up after injection during 12 months:the best corrected visual acuity(BCVA),the largest retinal pigment epithelial detachment height(hPED),the retinal pigment epithelial detachment area(aPED),the retinal pigment epithelial detachment volume(vPED)and the central fovea thickness(CFT).The changes of various data of 12 months before and after treatment were compared by paired t-test.The relationship between BCVA and hPED?aPED?vPED?CFT were compared by spearman's analysis.At the same time,the effect of the treatment methods on the prognosis of the disease was also discussed.ResultsAfter 12 months follow-up visits,twenty-two patients were found to have different degrees of improvement except for 1 patients who had not had any effect,all the patients with no major complications such as endophthalmitis?uveitis and RPE tears.The mean number of injections during the study period was 4.39±0.26.After 12 months the BCVA of ETDRS letters was raised from baseline 46.7±0.58 to 57.5±0.28.The mean maximum retinal pigment epithelial detachment height(hPED)was reduced from 350.17±35.73um to 238.87±36.87um,the mean retinal pigment epithelial detachment area was decreased from 5.12±0.69 mm2 to 2.74±0.89 mm2 and the mean retinal pigment epithelial detachment volume(vPED)was reduced from 0.81±0.21mm3 to 0.34±0.1mm3 after treatment(P all<0.05),The central fovea thickness(CFT)was changed from 324.56±16.42um to 286.85±18.39um after 12 months follow-up,but the difference was not statistically significant(P>0.05).There was no relationship between the BCVA and hPED?aPED?vPED?CFT(P>0.05).ConclusionsIntravitreal injection of Ranibizumab can treat wet age-related macular degeneration with serous pigment epithelial detachment.Using 3+prn injection can significantly reduce the degree of retinal pigment epithelial detachment,decrease the patient's central fovea thickness,and improve the patient's visual acuity in a safe and effective manner.As a safe method with little complications,it is worth popularizing.
