Objective:To Evaluate the function of proteomics in the diagnosis and differential diagnosis of Sarcoidosis, and thereafter help to find out new biomarkers in the future.Methods:Surface enhanced laser desorption ionization-time of flight-mass spectrometry was used to screen the serum of sarcoidosis and control patients. After statistical analysis and BPS cluster analysis, significant peaks related to the diagnosis of sarcoidosis, the differential diagnosis with tuberculosis and the activity of Sarcoidosis were found out, with their clinical effectiveness evaluated. The clinical information of Sarcoidosis and tuberculosis patients were analyzed and the effectiveness of usual methods such as SACE, PPD, TB-Elispot, CD4/CD8and characteristics in HRCT or bronchoscopy were evaluated, which were then compared to proteomic methods.Results:Seven nodes combined of M579776, M114833, M158348, M129649, M164812, M160734and M149790had the sensitivity and spectivity of94.87%, better than any methods used nowadays. Key node M114833was part of serum amyloid A, while M579776was unkown. Key node M137520was much higher in sarcoidosis than tuberculosis, which would help for their differential diagnosis. Peaks M103802, M187219, M196753and M941199were significant between groups before and after corticosteroid therapy of sarcoidosis, which might related to the actitivity of disease. Combination of several clinical indexes was still needed for the differential diagnosis of sarcoidosis and tuberculosis.Conclusions:Proteomics could improve the clinical diagnosis and differential diagnosis of sarcoidosis, which will also help to find biomarkers in the future.
