Abstract/Summary:
Objective: Obstructive sleep apnoea (OSAS) is a common disorder in whichrepetitive apnoeas expose the cardiovascular system to cycles of hypoxia, exaggeratednegative intrathoracic pressure, and arousalsSeveral studies show thatsleep disorderedbreathing is common in both men (4%) and women (2%). Lots of evidences have shownthat,OSAS may be the independent risk factor of cardiovascular diseases, such ashypertension, arrhythmia,Heart faliur, dyslipidemia and dibetes. OSAS also may lead oraccelerate the development of target organ damage, such as heart damage, endothelialdysfunction, lipid liver, stroke and coronary heart disease. So, it has been reported thatOSAS should be a new risk factor of cardiovascular diseases, and also be the part ofmetabolism syndrom. It should not be negetive that OSAS has affact human being’s heathseversely. Invetigating the characteristics of OSAS subjects and identifying therelationship between OSAS and cardiovascular diseases and target organ damage is veryimportant for prevention of negetive effect of OSAS. In addition, OSAS is an mutifactordisease. Genetics is the one of pathology of OSAS. Inflammation is associted with OSAS,many inlfammatary factors such as TNF-a are related with the development of OSAS.MK2gene is one of major regulation gene of TNF-a. In the present study, to analyze theclinical characteristics in2244OSAS subjects, and to study the association between genevariants in MK2and OSAS. Method:(1) total subjects is2244,554of women and1690of men. According to AHI, the subjects were divided into two group, hypertension groupand OSAS group which was divided into three subgroups(mild OSAS, moderate OSASand severe OSAS).(2) To analyze the cluster risk factor of cardivascular diseases amongfour groups. To study the relationship of OSAS severity and insulin resistance and targetorgan damage.(3) To perform the genotyping of MK2by Taqman RT-PCR. To analyze the association betweent sleep quality and SNPs. Results:(1)the rate of Smoking,dyslipidemia, diabetes and obesity in hypertensive subjects with OSAS are significantlyhiger than that in hypertensive subjects without OSAS(P<0.05). The cluster risk factor ofcardiovascular disease is related to severity of OSAS, expecially to noctunal oxygensaturation.(2)the rate of glucose metabolism disorer is significantly higher in hypertensivesubjects with OSAS are significantly higer than that in hypertensive subjects withoutOSAS.(3) The rate of LVH is significantly higher in OSAS group than in hypertensiongroup. OSAS severity is the independent risk factor of carotid atherosclerosis.(4) G alleleof rs11119385and A allele of rs4844550in MK2is associated with sleep quality.Conclusion:(1) Patients of hypertension with OSAS has larger risk of cluster ofcardiovascular diseases.(2) OSAS increased the risk of insulin resistance in hypertensivepatients.(3) OSAS deteriorate the target organ damage in hypertensives.(4) rs4844550and rs11119385in MK2are the independent risk factor of decreased sleep quality.
