Background and objective:The acknowledge and treatment of traditional Chinese medicine on infectious diseases has been clinically confirmed over the years. However, traditional Chinese medicine for severe infections, such as air temperature Hyperactivity phlegm obstruct the lung disease (severe lung infection) lacks a clear and stable effect. In recent years, Bai-Hu-Ren-Sen-Tang treating exopathic fever and compound Xie-bai capsules treating chronic obstructive pulmonary disease and acute exacerbation of bronchitis have clear efficacy. However, its function and mechanism on air temperature Hyperactivity phlegm obstruct the lung disease was unclear. This study aimed to investigate the the effect, prognostic evaluation system and molecular mechanism of this prescription on treating the air temperature Hyperactivity phlegm obstruct the lung disease (severe lung infection). We evaluated the clinical efficacy of based control groupand traditional Chinese medicine treatment group by body temperature, symptoms, physical examination and other indicators; measured miRNA expression profiles in peripheral blood, evaluated the response to changes in the peripheral blood of miRNA after treatment, and explore the possibility of Bai-Hu-Ren-Sen-Tang and compound Xie-bai capsules on treating Air temperature Hyperactivity Disease (Hyperactivity Fushi). And through this study, provide new targets for the evaluation of the efficacy of traditional Chinese medicine, and to determine the prognostic value in the evaluation, in order to study the role of the principles of Chinese medicine to open up new ideas and methods.Methods:The basic control group was treated with the necessary antibiotics and adjuvant therapy, while Chinese medicine treatment group were given Bai-Hu-Ren-Sen-Tang and compound Xie-bai capsules on the basis of treatment. the changes of body temperature, white blood cells and neutrophils, the endogenous pyrogen including IL-1β, TNF-α, IL-6, and IFN-α and γ, etc. before and after the treatment in the two groups were compared, and proceeded by a comprehensive evaluation of the clinical efficacy of traditional Chinese medicine. Further experiments of in vitro anti-bacteria showed Bai-Hu-Ren-Sen-Tang and compound Xie-bai capsules had effecively antibacterial effects on Staphylococcus aureus (ATCC25925), Streptococcus pneumoniae, Escherichia bacteria, Pseudomonas aeruginosa, which indicated a clear relationship between the compound preparation and treatment effects. Using high-throughput miRNA biology chip testing, the peripheral blood miRNA changes were analyzed before and after Bai-Hu-Ren-Sen-Tang and compound Xie-bai capsules treating the air temperature Hyperactivity phlegm obstruct the lung disease (severe lung infection).This study aim to investigate the efficacy of traditional Chinese medicine mechanism from the perspective of miRNAs, and establish a prognostic evaluation system with miRNA.Results:1. Bai-Hu-Ren-Sen-Tang and compound Xie-bai capsules could effectively inhibit the fever of patients. After t test, P value of treatment group compared with the control group was<0.01, which explained Bai-Hu-Ren-Sen-Tang and compound Xie-bai capsules had significant antipyretic effects and significantly improved Chinese clinical symptoms integral in patients; and reduced white blood cell and neutrophil counts; 2. The endogenous pyrogens including interleukin-1 (interleukin 1, IL-1) β, TNF (tumor necrosis factor, TNF) a, interleukin-6 (interleukin 6, IL-6), and so on, were significantly inhibited after giving Bai-Hu-Ren-Sen-Tang and compound Xie-bai capsules compared with the control group (P<0.01).3. In vitro antibacterial experimental observations showed that Bai-Hu-Ren-Sen-Tang and compound Xie-bai capsules could significantly inhibit the growth of Staphylococcus aureus pneumonia meningitidis, Haemophilus influenzae, Escherichia bacteria, indicating that the drug has significant antibacterial effects.4. miR-146a, miR-125b and miR-155 in peripheral blood before and after treatment of Bai-Hu-Ren-Sen-Tang and compound Xie-bai capsules have significant changes.Conclusion:Bai-Hu-Ren-Sen-Tang and compound Xie-bai capsules have detoxification, anti-inflammatory, antibacterial effects, which supplied the foundation for the development of new drugs. miR-146a, miR-125b and miR-155 can be used as treatment indicator for prognosis evaluation in patients treated with Bai-Hu-Ren-Sen-Tang and compound Xie-bai capsules.
