Objective: This study were analyzed to evaluate whether the aneurysm wasan important factor to left ventricular function, the relationship between the sizeof aneurysm and left ventricular function, the effects of surgical treatments andsurvival rates postoperation through cardiac functional factors. Early, mid-termand long-term qualities of life were other methods to evaluate the effects ofsurgical treatments. Factors with surgical treatments were analyzed to evaluate theeffects in the operation. Methods:we assembled105patients who were received surgical treatmentsof left ventricular aneurysm during1998-2014in our hospital, including94malesand11females, age were62.69±10.26years, weight were72.9±11.5kg.Thesewere3cases received Cooley surgical treatments,14cases received Jantenesurgical treatments,88cases received Dor and SAVE surgical treatments. Weanalyzed statistically and comparatively left ventricular functional factors thatwere including left ventricular diameter, wall thickness, left ventricular end-systolicvolume and end-systolic volume index, left ventricular end-diastolic volume andend-diastolic volume index, ejection fraction, left ventricular end-systolicsphericity index, left ventricular end-diastolic sphericity index, eccentricity indexand circular shape index, cardiac function, these cardiac function factors wereobservated before operation and postoperation2weeks,1year,3years and5yearsfrom surgical treatments through echocardiogram, computerized tomographicscanning and magnetic resonance imaging. The survival rates, qualities of life andfactors with surgical treatments were also viewed after1year,3years and5yearsof operation. Results:1.Ejection fraction, left ventricular diameter and left end-systolic volumeindex were the main influential indicators of cardiac function. The cardiac functionwas improved after the operation with those cardiac functional factors beforeoperation and postoperation2week,1-year,3years and5years, which wasstatistically significant (P <0.05).The result confirmed that the operation of leftventricular restoration was not only reshapd the ventricular but also improvecardiac function.2.We investigated the qualities of life after the operation of1year,3yearsand5years postoperation, the qualities of life postoperation were better thanpreoperation and the differences were statistically significant (P <0.05). Theopeartion of left ventricular aneurysm resection apparently improved the patients’qualities of life.3.Patients’ survival rate was (82.91±4.20)%postoperation5years withKaplan-Meier method and it was better. That mean our surgerical outcome wassatisfactory. There were the most important factors for left ventricular end-systolicvolume index、left ventricular end-systolic eccentricity index and left ventricularend-diastolic circular shape index to the survival rate by COX survival analysisshowed. It helped to restore ventricular cone structure, improve heart function andimprove survival by reconstructing the ventricular cavity to normal volumethrough comparely analysing those factors and survival rates.Conclusion:1. We analyzed the cardiac factors that was reflected the changes ofstructure and function and found ejection fraction, size of aneurysm and leftend-systolic volume index were the main influential indicators and the changes ofstructure was more important than volume for cardiac function. Those revealedthat the operation of left ventricular restoration was not only reshapd theventricular but also improved the cardiac function..The qualities of lifepostoperation were better than preoperation, it mean the opeartion of leftventricular aneurysm resection apparently improved the patients’ qualities of life. 2.Patients’ survival rate was (82.91±4.20)%postoperation5years in ourgroup and it was better than the others. That mean our surgerical outcome wassatisfactory. There were the most important factors for left ventricular end-systolicvolume index、left ventricular end-systolic eccentricity index and left ventricularend-diastolic circular shape index to the survival rate. Our surgical treaments notonly restorat the ventricular conical structure but also improve heart function andsurvival rates for patients through comparely analysing those factors and survivalrates.3. left ventricular systolic eccentricity index and left ventricularend-diastolic Gibson index which reflected the changes of structure can be used asnew factors to evaluate cardiac function and survival rates, the effects of surgicaltreatments and provide references for the operation in the future.
