Purpose:1. Further segmentation of blood vessels, tumors in the liver; preliminary studyof virtual bile duct in the three-dimensional visualization.2.To explore the effectiveness of the precise treatment planning of microwave ablation to middle and large size liver cancer by using three-dimensional visualization.3. Preliminary research of treatment planning of microwave ablation to middle and large size renal cancer by using three-dimensional visualization, to explore the feasibility of treatment planning to middle and large size renal cancer.Materials and methods:1. Level set method is adopted to further segmentation research of liver blood vessel, tumor; Explore virtual bile duct technologyin liver cancer ablation before treatment planning.2. The study on treatment planning of the microwave ablation to liver cancer: From November 2013 to December 2014, according to enrollment standard,112 cases (128 nodules) were enrolled in a randomized control study to middle and large size liver cancer. All cases were randomly divided into two groups according to admission order. In the three-dimensional treatment planning group,58 cases (64 nodules) underwent the treatment planned by the 3D visualization software before MWA. In the no-planning group,54 cases (64 nodules)underwent the treatment planned by the 2D medical images before MWA.To adopt for the treatment planning to irregular large liver cancer, more times of insertion were admitted. The items of these two groups were compared prospectively includinginsertion number, treatment time, success rate of first ablation, technique effectiveness rate and local tumor progression ratepostoperative 1 month.3. Preliminary discussion on application of the three dimensional visualization treatment planning in middle and large size renal cancer:From April 2013 to March 2015, treatment planning was applied to 14 cases of large renal cancer to discuss the feasibility of treatment planning.Results:1. Using the level set method can more accurately and quickly segment the liver vessels, tumors.Virtual bile duct technology can simulate forming virtual bile duct, provide bile duct structure of three-dimensional visualization.2.Research of precise treatment planning to liver cancer:precise treatment planning were successfully performed in group. By using precise treatment planning, insertion number and treatment time in the 3D treatment planning group were more than those in the no-planning group (P<0.05). Local tumor progression rates in 3D treatment planning group and tumor more than 5cm 3D treatment planning subgroup is lower than that in the no-planning group(P<0.05).3. The coincidence rate of precise treatment planning to large renal cancer is 75% from 50% in the early,3D visualization software can be done in treatment planning study to middle and large size renal cancer.Conclusion:1. Level set segmentation study can quickly accurate segmentation to tumor and vessels, virtual bile duct technique can be used to the 3D visualization in treatment planning.2. Using three-dimensional visualization software, precise treatment planning can provide precise ablationto middle and large size liver tumor in reducing local tumor progress rate.3. Using three-dimensional visualization software, treatment planning is feasible for middle and large size renal cancer.
