A global epidemic of hepatitis C virus infection is increasingly becoming a huge global public health problem. the prevalence rate of HCV infection in most countries is in the 1%between2% but China has reported a number of different HCV prevalence rate,3.2% of which is the most recognized. HCV is very serious impact to human health.They prove the existence of an association between clinical results of HCV treatment and SNP since 2009. Studies have shown that polymorphisms of the vicinity of interleukin 28B (IL28B) gene can be predicted so the patients with HCV genotype Ⅰ could get rid of the virus with HCV treatment, but we are short of datas of Chinse people.The pathogenesis of hepatitis C is complex and may be related to the following factors: ① the cell mediated immune injury; ② apoptosis: Fas mediated apoptosis is a form of liver cell death with HCV infection; autoimmune: HCV is often associated with the characteristics related to immunity. Some studies have manifested that people who using interferon to treat HCV will increasea the incidence of the thyroid dysfunction, but the relevance has no data support among Chinese population.This research was designed to collect data of HCV patients in China, to acquaint the distribution of SNPs of IL-28B in China along with the analysis of correlation between the patients with HCV complicated with TD and geographic distribution, host age, host gender, HCV genotype and RNA replication, so as to provide relevant clinical data obtained strong epidemiological evidence to better understand the disease, clinical research fellows to develop individualized antiretroviral therapy programs for different groups. Part Ⅰ: the research of epidemiological investigation on China’s chronic HCV genotype distribution and its correlation between interleukin 28B gene polymorphismObjective:To investigate the correlation HCV infected genotype and geographic area in china, host, age, and RNA levels of replication; to Analysis the distribution of IL-28B in China,and HCV genotype and IL-28B SNPs correlation in order to clarify the relationship between IL-28B SNP and HCV genotype of hepatitis C in china.Methods: Selecting a total of 1014 cases of HCV infection in 28 hospitals from different regions all over China, choose the index of gender,age, complications, HCV genotype, RNA level to statistical analysis, the gene frequency of HCV types with 13 species of IL-28B SNPs were investigated.Results:1. Most of Chinese patients has HCV genotype Ⅰ. HCV gene type Ⅰ and type Ⅱ are the main genotypes in the most parts of China; while HCV genotype of type Ⅰ and type Ⅲ distribution is higher in southwest and south region.2. In the young group, who was compensation period are very rare. It is more familiar in middle-aged group.And male patients are more likely to suffer from than female in the middle-aged group,3. There is no gender difference in all regions of the HCV genotype Ⅰ (P< 0.05). In the china of parts of southwestern, men are significantly less than female patients with HCV gene type Ⅱ (P< 0.01); HCV gene type Ⅲ, except the northeast of China, men are widely to suffer from in all areas; China has more men than women with HCV gene type VI in the south area (P< 0.01).4. Except of the HCV genotype Ⅰ b, RNA is very level replication in of all others. The genotype of HCV Ⅰ, Ⅱ, Ⅲ has no significant difference between HCV-RNA expression level.5. The 13 kinds of IL-28B SNPs loci frequencies vary, as most of the SNPs in HCV gene between type Ⅰ and Ⅱ has significant difference in the proportion of. All SNP genotypes.Conclusion:CHC with different genotype were lived distribution in different areas of China, which has a explicit relation in age, gender; proportion between the various SNPs IL-28B in the HCV virus genotype Ⅰ and Ⅱ differences. Part Ⅱ:The study on the effect factors of Chinese CHC with abnormal thyroid function of epidemiologicalObjective:This study researched the correlation of the TD and the untreated CHC patients, and the influential factors of HCV combined with TD to provide accurate information for clinical treatment.Methods:selecting a total of 1014 cases of HCV infection in 28 hospitals from different regions all over China, statistic analysis was performed on the index of thyroid function, region, age, gender, genotype, and RNA.Results:1. In terms of nationwide, people who infect HCV are more easily combined with TD in north. TD types varies from each area. Subclinical hypothyroidism in north, northwest, northeast, southwest is very more common; while Hyperthyroidism combined with chronic thyroiditis has higher proportion in the south area.2. The high HCV-RNA replication in HCV patients are most common, but the level of HCV-RNA replication is not relavent to the Morbidity of TD.3. The CHC with TD is most in middle-aged; the female patients who have HCV with a huge incidence of TD.4. In total HCV genotypes, type Ib, II are the very most common, but there is not obvious correlation between the HCV genotype and the incidence of TD.5. In HCV patients, TD group has a higher rate of abnormal liver function than non TD group, therefore, there is a certain amount of correlation between the liver function and TD.Conclusion: This study shows that HCV infection induced TD and its type in china have a relation of geographical distribution, gender, age, HCV-RNA level, but have no very significant correlation with genotype.
