Along with the development of social economy, the living standard of peopleand health care have been improved continuously, and the requirement for improvinglife quality becomes more intense, certainly total joint replacement surgery hadalready been introduced successfully in China, and it’s clinical application providesan effective treatment. China is abundant in population, with the rapid increase of thenumber of total joint replacement, how to increase therapeutic effect on THA anddecrease surgery faults turn to a subject worth exploring for biomedical researchers.The present joint implants are design by most of American and European companiesaccording to Western standards. Clinical practice proved that Chinese people’sanatomical construction of joint, the living activities, the joint biomechanics aren’tidentical to that of the west race due to differences in the ethnic and culture betweenEast and West. So it is a very urgent task to improve the operation technique level ofhip joint replacement and developing the hip prosthetic which is suit for Chinesepopulations.This dissertation is based on the key international cooperation research project‘Research on the properties of hip and knee joints and basic design of artificial jointsfor Asia race’ supported by NSFC (No.30810103908), following a series ofexperiments and basic research on human hip joint, it can be the basic foundation tofurther development joint prosthesis which is suit for Asian populations. We makestatistical analysis and special studies for the common THA failure. The kinematicdata of lower limbs have been carried out for typical movement of Chinese people,statistical analysis is made for these data. A network-based shareable human motionanalysis software has been developed. A new hip motion analysis method is presented, and a new practical hip motion analysis software has been developed. We also makeanatomical measurement and statistical analysis at hip for Chinese people. Based onmeasured kinematics data and the hip anatomic data above mentioned, we used hipmotion analysis software to analyze theory mobility of hip for Chinese and Westpeople. According to the same methodology, and we make analysis to investigateTHA’ impingement/dislocation risk under common living activities. From the actualclinical need, we studied the range of motion (ROM) of THA under different implantsite. The main contents are listed as following:1) Statistical analysis of real data on common failures of total hip arthroplasty hadbeen done, and an in-depth study of the four most common failures (loose,dislocation, wear, fracture) had been done also, the specific thinking path ofclinical diagnosis were proposed respectively.2) The optoelectronic3D motion tracking system was used to capture theanthropometric kinematics for7most common living activities: normally walking,jogging, climbing the stairs up/down, squatting, kneeling and sitting cross-legged.The kinematic data of the lower limbs of50Chinese peoples was collected. Thedata was analyzed statistically, the means of the3D angles of the ankle, knee, andhip joints of these typical activities were calculated. The data could be valuablefor improving operation effectiveness, rehabilitation protocol and establishingcriteria for lower limb prosthetics for the Chinese population. In order to save andshare these data, a human kinematics database software based WEB wasdeveloped, and kinematic data would be shared easily (The certificate number:2013SR066383).3) A digital measure method for the anatomic parameters of hip was presented,three-dimensional geometry models were constructed with professional medicalsoftware based CT image data, the anatomical measurement of the hip wasstudied in health Chinese adult. The differences had been found in some parts andthe parameters comparing the west race on the basis of statistical analysis. Thesedata can provided anatomical basis for clinical treatment of joint and thedesigning of artificial joint. 4) A new hip motion analysis method was presented, and a hip motion analysissoftware was developed correspondingly. The software has friend interface,functionality for visualization of results is also provided, and it can provides thepractical help to design THA and implant, also can predicts the THA’ mobility,so it has a good clinically application in future.5) Base on the results obtained by above techniques, the relationship between THA’impingement/dislocation and6typical activities were investigated with theapplication of the hip motion analysis software. Then the relationship betweenTHA’ impingement/dislocation and the implant site was researched also. Theresults can provide scientific, effective and theoretical basis for the improvementof the surgical treatment of joint replacement and the design level of jointimplants.Anyway, in order to enhance effect of Chinese’ total joint replacement, thispaper analyzed the main failure forms of total hip arthroplasty and studied thekinematics of lower limbs and the anatomical measurement of the hip joint. Thesefundament research provided the theoretic foundation and technical data for designingthe hip prosthetic which is suit for Chinese populations. Presented a new hip motionanalysis method, and developed a software base this method correspondingly, usingthis software to analyze and evaluate THA’ dislocation risk under the common liveactivities and the different implant sites, these results give some experiences anduseful information supports for the researchers in the related fields.
