Objective:To investigate the volume change and weight change at different in vitro time by empirical study and clinical study. Analyze the volume and weight change rate to acquire the effect of autogenous costal cartilage in vitro time on the volume and weight changes of autogenous costal cartilage. Observating the volume and weight changes of allogeneic costal cartilage acellular matrix by empirical study to accumulate experiences for allogeneic and heterogeneous costal cartilage acellular matrix graft.Methods:1、The harvested autogenous costal cartilages were cut into similar length small blocks and divided into20groups by the in vitro time and embedded time in body, then the costal cartilage blocks were transplanted subcutaneously. Measuring the volume and weight of the costal cartilage blocks before the transplantation and after the removal, analyze the volume and weight change rate of the costal cartilage blocks. Morphological and ultrastructure changes of autogenous costal cartilage were observed through light and electron microscope by image analysis system.2、Transplanting the remaining pieces of cartilage after the costal cartilage framework fabrication in ear reconstruction under the skin of the chest. The patients were divided into two groups by the differences of in vitro time. Measuring the volume of the costal cartilage pieces before the transplantation and after the removal, analyze the volume change rate of the costal cartilage pieces of the two groups. Morphological changes of autogenous costal cartilage were observed through light microscope by image analysis system.3、The allogeneic costal cartilage acellular matrixes were implanted into pigs subcutaneously for one or two months, and then they were taken out. They were divided into two groups by the differences of their embedded time in body. Measuring the volume and weight of the allogeneic costal cartilage acellular matrixes before the implantation and after the removal, analyze the volume and weight change rate of the allogeneic costal cartilage acellular matrixes and compare the volume and weight change rate of the two groups. Morphological changes of allogeneic costal cartilage acellular matrix were observed through light microscope by image analysis system.Results: 1、There were no statistically significant difference in volume and weight change rate of autogenous costal cartilage graft among different embedded time in body under the same in vitro time before transplantation.2、There were no statistically significant difference in volume and weight change rate of autogenous costal cartilage graft between1hour in vitro time and2hour in vitro time before transplantation. There were statistically significant difference in volume and weight change rate between3~5hour in vitro time and1~2hour in vitro time.3、There were no statistically significant difference in volume change rate of human autogenous costal cartilage graft between0~1hour in vitro time and1-2hour in vitro time before transplantation.4、The images of autogenous costal cartilages observed through light microscope and electron microscope showed that the degree of cell degeneration and necrosis of autogenous costal cartilages increased with the increase of in vitro time. The degeneration and necrosis were most obvious in the center of the costal cartilage. It has nothing to do with the embedded time in body. There was no obvious structural changes of allogeneic costal cartilage acellular matrix after implantation.5、There were no statistically significant difference in volume and weight changes before and after implantation. There were no statistically significant difference in volume and weight change rate between different embedded time in body.Conclusions:1、There was no obvious differences in volume and weight change rate with in2hour.2、There was obvious differences in volume and weight change rate between with in2hours and more than3hours.3、Different degrees of cell degeneration and necrosis will occur while the in vitro time of costal cartilage, and will exacerbate as the time increasing, especially after2hours.4、Embedded time has nothing to do with the volume and weigh changes of cartilages graft.5、There was no obvious absorption after allogeneic costal cartilage acellular matrix graft. It shows that allogeneic costal cartilage acellular matrix graft is available. Further research is needed.
