Objective:Purpose with the teacher:Through intensive study of classical medical science based theory, to enhance and expand our comprehension in the theory of traditional Chinese medicine, to deepen the foundation of traditional culture, so as to promote the differential diagnosis and treatment according to the theory of traditional Chinese medicine, to collect, summarize and inherit the academic thoughts, clinical experiences and skills of the instructor after the three-year clinical study and independent clinical practice.Research purposes:To evaluate the efficiency of Gukang from different aspects by studying the changes of bone mineral density, bone histomorphometry and bone metabolism of animals with postmenopausal osteoporosis, and by analyzing bone mineral density, FRAX assessment, leukocyte telomere length and bone metabolic markers these four indexes in patients with postmenopausal osteoporosis.Method:Learning methods:Mainly self-study, assisted by concentrated instruction made by teacher, including online learning of the four classic courses. Under the guidance of Professor Liu Qingsi, in-depth study, comprehension and exploration of the essence of classical literature was done, notes and experieces were taken down. In the three years of clinical learning from the instructor and independent clinical practice, learning notes, follow-up medical records, operation records and experiences achieved were written down and comprehended cautiously, in a hope to inherit instructor’s academic thoughts and improve the writer’ s clinical skills.Research methods: (A)Animal experiment:486-month-old healthy female SD rats were randomized into4groups,12in each group:control group, model (OVX) group, traditional Chinese medication (TCM) group and western medication (Western) group. Lavage for12weeks was performed after the establishment of models. Weight of rats before and after sacrifice, bone mineral density, bone histomorphometry and bone metabolism changes of the femur in vitro were analyzed.(B)Clinical study:60postmenopausal osteoporosis cases eligible for inclusion were enrolled and divided into Gukang treatment group (GK) and Xianling Gubao group (XLGB), with their bone density test, FRAX assessment, leukocyte telomere length test and biochemical markers of bone metabolism changes analyzed.Results:Achievements following the teacher:The writer got excellent remarks in theoretical study, accomplished the follow-teacher-study and independent clinical practice sufficiently and efficiently according to the requirements of the State Administration of Traditional Chinese Medicine, PRC.11summaries on experience of classical medical science had been submitted, more than180notes had been taken down, and records of60medical cases were sorted.3papers on analyzing and summarizing Professor Liu Qing-si’s academic thoughts and skills had been published in high-quality journals already, and another paper is to be published soon.Results:(A) Animal experiment:1. It is found that in ovariectomized rats, weight significantly increased but the bone mineral density of left femur decreased, which is similar with the changes of postmenopausal women, indicating that post-menopausal osteoporosis modeling is successful.2. It was found that both GuKang and estradiol valerate may inhibit weight gain in ovariectomized rats (P<0.05), improve the density of left femur in rats (P<0.05), significantly upgrade serum E2, BGP,1,25-(OH)2D3and CT levels (P<0.05), and degrade PTH levels (P<0.05).3. Significant difference of bone formation indexes including the percentage of labeled perimeter and bone formation rate indicators was noted in ovariectomized rats of traditional Chinese medication (TCM) group and western medication (Western) group comparision with control group and model (OVX) group.(P<0.05).(B) Clinical findings:1. According to dynamic observation of bone density in Gukang and Xianlinggubao treated patients, it was noted that their bone mineral density was significantly higher after treatment (P<0.05).2. Our dynamic observation showed FRAX had no significant reduction after intervention in both groups (P>0.05).3. Leukocyte telomere DNA length changed significantly in Gu kang treated patients (P>0.05).4. β-Cross decreased but PICP and N-MID increased significantly in Gukang treated patients.Conclusion:With the teacher Conclusion:Three years of study has been successfully completed. In the past three years, I devoted myself to comprehensively studying and summarizing kidney and spleen invigoration and blood promotion for bone atrophy, the academic thoughts of Professor Liu Qingsi, applied and promoted these in my independent clinical practice, with satisfactory effect achieved. Besides medical science, I also gained a lot and rather moved for my teacher’s devoted, diligent and rigorous style, and I will take him as my lifelong model. The three years of studying under my teacher is coming to an end, but never will the road of traditional Chinese medication since we will inherit it. The task is still heavy, but we are ready to take the opportunity, to assume the responsiblity faithfully to take the road of Chinese medicine, so as to make greater contributions to promoting our traditional medication.(A)Conclusion from animal experiment1. Ovariectomized rats had decreased bone mineral density but significantly increased body weight.2. Effect of Gu kang was similar with estradiol valerate, both of which inhibit weight gain and increase femoral bone mineral density in ovariectomized rats.3. Both Gu kang and estradiol valerate could significantly increase serum E2, BGP,1,25-(OH)2D3and CT levels, and decrease PTH levels, indicating that GuKang has similar effect with estradiol valerate. Since GuKang contains estradiol-like substances with estrogen-like effect, it may act directly on estrogen receptors, osteoblasts and osteoclasts, inhibiting bone resorption and promoting bone formation.4. Gu kang may improve bone turnover indicators represented by the percentage of labeled perimeter and bone formation rate, implicating that Gu Kang can effectively improve the high bone conversion rate in ovariectomized rats, similar to estrogen.(B)Clinical conclusion1. In the dynamic observation of osteoporotic patient gold standard for bone density and from the perspective of iconography, it is noted in the imaging perspective that bone mineral density significantly increases after Gu Kang intervention.2. According to our dynamic macroscopic analysis of the probability of fracture, the most serious complication in patients with osteoporosis, fracture risk10years later after Gu kang intervewntion may not significantly reduce.3. In the simple dynamical observation, it was noted that Gu Kang may change the leukocyte telomere DNA length from the perspective of molecular biogenetics.4. In dynamic observation of bone formation indicators, it was observed that Gu Kang may reduce osteoclast activity, weaken bone resorption, enhance bone formation and bone turnover.
