Objective To explore the characteristics about depression and sleep-disorder in PD. It will provide the reference for the early diagnosis of PD. To investigate the differentiation from rat adipose mesenchymal stem cells to neuron-like cells by Y-27632.Materials and Methods Clinical part:318PD patients in outpatient were investigated by questionnaire. These PD patients were extrapyramidal system outpatients in Neurology Department of Tianjin Medical University General Hospital. The diagnosis of PD was made according to the United Kingdom Parkinson’s Disease Brain Bank Criteria. The contents of the questionnaire include General Information, Zung’s Self-Rating Depression Scale, Mini-Mental State Examination, the Pittsburgh Sleep Quality Index, Unified Parkinson’s Disease Rating Scale. Experiment part: Different Y-27632concentration induced changes in rat adipose mesenchymal stem cells were observed with an microscope linked to a camera. Expression of nestin, NSE, Map-2and GFAP were determined by Immunocytochemistry. The data were statistical analysis by SSPS17.0.Results Clinical part:①The incidence about PD patients with first symptom of depression is11.64%。Age, depression degree, duration of PD diagnosis is different between PD patients with first symptom of depression and others.②the score of each SDS item was higher in our PD patients than in the general Chinese population. The female, aged, living in village, poorly educated, long disease duration, depression before PD diagnosis, higher UPDRS and Hoehn&Yahr had more moderate and major depression.③When some influence factors were analysed by Binary logistic regression analysis, education degree has negative correlation with PD depression, UPDRS Ⅰ and disease duration has positive correlation with PD depression.④In this study gender and education relate to sleep-disorder in PD.⑤PSQI scores about subjective sleep quality, sleep latency, sleep duration, habitual sleep efficiency, sleep disturbances, use of sleep medication, and daytime dysfunction over the last month were higher in our PD patients than in the general Chinese population.⑥When some influence factors were analysed by Binary logistic regression analysis, sex, UPDRS I score and UPDRSⅢ score had positive correlation with sleep-disorder in PD. Experiment part:①Y-27632can promote AMSCs changed into neuron-like cell with higher concentration and time extension;②Y-23632can induce AMSCs express nestin, NSE, MAP-2.Conclusion Clinical part①The incidence about PD patients with first symptom of depression is11.64%. PD patients with first symptom of depression are at a young age, with serious depression. PD patients with first symptom of depression has a long time to be diagnosed。②In the present study the incidence of PD depression was65.4%, female, aged, living in village, poorly educated, long disease duration, depression before PD diagnosis, higher UPDRS and Hoehn&Yahr has an influence with depression degree.③PD patients with poorly educated, higher UPDRS Ⅰ and long disease duration was at risk for PD depression.④In the present study the incidence of sleep-disorder in PD was67.3%, female and poorly educated PD patients has worse sleep quality.⑤Female PD patients and with higher UPDRS Ⅰ score and UPDRSⅢ score PD patients were at risk for sleep-disorder. Experiment part: Y-27632can induce AMSCs differentiation into neuron-like cell in vitro.
