Comparison Study Of Metabolic Syndrome’s Differences And Diagnostic Criteria’s Applicability Among Xingjiang Uigur, Kazak And Han Population

Objective: To investigate the prevalence of metabolic syndrome (MS) among Xingjiang Uigur, Kazak and Han population and compare the differences and applicability of three diagnostic criteria used for MS, The three diagnostic criterias are the National Cholesterol Education Program (ATPⅢ), International Diabetes Federation (IDF) and China Diabetes Society (CDS). Methods:A cross-sectional study was conducted on 2053 Uigur, 2219 Kazak and 2659 Han people of Xingjiang aged 30-80. The prevalence of MS and the degree of agreement were both calculated according to the three definitions and the results of MS components were also analyzed. Results: 1) With regard to Uigur population, according to the three definitions, the prevalence (adjusted prevalence) of MS were 35.80% (29.64%), 39.41% (35.88) and 23.72% (19.17%), the prevalence of subjects presented at least 3 risk factors were 100%, 97.90%and 92.81%, among all the MS subjects, The MS diagnostic criteria of ATPⅢand IDF were in good accordance with Kappa index (0.7928) and concordance rate (0.9026), CDS was in relatively weak agreement when comparing with other deftnitions (ATPⅢand IDF). 2) With regard to Kazak population, according to the three definitions, The prevalence (adjusted prevalence) of MS were 23.21%(21.29%), 23.88%(21.96%) and 18.21% (16.65%), the prevalence of subjects presented at least 3 risk factors were 100%, 96.23%and 91.34%, Among all the MS subjects, the MS diagnostic criteria of ATPⅢand IDF were in good accordance with Kappa index (0.9663) and concordance rate (0.9878). CDS was in relatively weak agreement when comparing with other deftnitions (ATPⅢand IDF). 3) With regard to Han population, according to the three definitions, the prevalence (adjusted prevalence) of MS were 27.03%(27.18%), 22.40%(20.39) and 16.04% (16.02%), The prevalence of subjects presented at least 3 risk factors were 100%, 98.49%and 89.91%, among all the MS subjects, the MS diagnostic criteria of IDF and CDS were in good accordance with Kappa index (0.0953) and concordance rate (0.7172). ATPⅢwas in relatively weak agreement when comparing with other deftnitions (CDS and IDF). 4) According to the three definitions, ATPⅢdiagnostic criteria’s morbidity rate (adjusted morbidity rate) of MS was the highest for Uigur and Kazak population, they were 39.41% (35.88%) and 23.88% (21.96%). ATPⅢdiagnostic criteria’s morbidity rate (adjusted morbidity rate) was the highest for Han population, it was 27.03% (27.18%); According to the three definitions, ATPⅢdiagnostic criteria’s morbidity rate of subjects presented at least 3 risk factors was the highest for Uigur (98.64%), Kazak (100%) and Han population (100%). CDS diagnostic criteria’s morbidity rate of subjects presented at least 3 risk factors was the lowest for Uigur (92.81%), Kazak (91.34%) and Han population (89.91%). The MS diagnostic criteria of ATPⅢand IDF were in good accordance with Kappa index, Youden index and concordance rate among Uigur and Kazak population. The CDS criteria and IDF criteria were in good accordance with concordance rate among Han population. Conclusion: 1) There was big differences in the prevalence, the degree of agreement and aggregation of risk components on MS among Xingjiang Uigur population. IDF criteria’s detection rate of MS was the most, ATPⅢcriteria’s detection rate of subjects presented at least 3 risk factors was the most, the concordance of ATPⅢcriteria and IDF was better. 2) There was big differences in the prevalence, the degree of agreement and aggregation of risk components on MS among Xingjiang Kazak population. IDF criteria’s detection rate of MS was the most, ATPⅢcriteria’s detection rate of subjects presented at least 3 risk factors was the most, the concordance of ATPⅢcriteria and IDF was better. 3) There was big differences in the prevalence, the degree of agreement and aggregation of risk components on MS among Xingjiang Han population. ATPⅢcriteria’s detection rate of MS was the most, ATPⅢcriteria’s detection rate of subjects presented at least 3 risk factors was the most too, the concordance of ATPⅢcriteria with CDS and IDF was weak. 4) The MS diagnostic criteria of IDF shoud be first choice for MS screening of Xingjiang Uigur and Kazak population. The ATPⅢcriteria shoud be first choice for Han population. ATPⅢcriteria was better than other two criterias for screening of subjects presented at least 3 risk factors among Xingjiang Uigur, Kazak and Han population. ATPⅢcriteria was better than other two criterias for screening MS among Xingjiang Uigur, Kazak and Han population.
