This dissertation mainly research in the field of medical protection mechanism in the rural China, particular about the institution of CMS. The research work consists of theoretical part and empirical one. If given space and time location, the empirical research is about two township CMS institutions in south of and north of Jiangsu province since 1990s, this two CMS is different phenomenally: one evolutes from 1950s and has long tradition, the other is constructed since 1992; one looks like alluding transformation, the other like compulsory change. Nevertheless, they all face with serious crisis.In order to analysis the problem, we would rather to search for some theoretical instruments before beginning our empirical research. To this topic, there is no mature sociological theory, even a systematical one, so I would have to put hands to the economics or political theory. If the feature of sociological research is comprehensive, empirical and scientific, this study is sociological one. Because CMS is known as one kind of primary insurance institution, we would research in the fields of institution analysis, social protection, medical insurance. In the aspect of institution analysis, we try to find fitful general method to analysis the institutional change of CMS. In social protection aspect, we want to find the principals of social security and social insurance, and what is fitful aim of health protection institution. In medical insurance, we attempt to draw an trying decision-making model of Chinese peasants.From view of game theory, the institution is the outcome of game of participants in condition of given conditions. Therefore, we investigate the three participants: CMS manager, rural medical servers and local people, which is the demander of medical service and CMS payer. After the investigation and analysis, the dissertation would give an explanation of CMS change in two different local contents.This dissertation falls into eight chapters.1st chapter is the introductory part, which is devoted to briefing the research backgrounds, status, intention, possible meaning.The 2nd chapter is about the topic, basic concepts and analysis frameworks of social protection mechanism. This chapter is the outline of the total research.The succeed 3rd chapter is about institution analysis; in this chapter we regard the institution as an system consisted institution circumstance, structure, function, organization, and participants. In this paper I try to combine the related views of institution analysis in economics, political economy, sociology to create a various dimension analysis.In 4th chapter, we rethink the social insurance, focused on the social principle and insurance principle, and its compulsory character.In the 5th chapter, we talk about the basic rule of insurance, medical insurance, medical insurance demand, and prove an empirical model, including five suppositions which reflect such factors as empiric anticipation style, residual risk, transition cost, alternative effect, excluding constrains.Then the succeed part is empirical research. The 6th and the 7th chapter are two case studies about CMS and medical protection of rural residents. These two chaptersare affluent phenomenally in research of the topics. Then the 8th chapter is the comparison of the two cases, both using quantitative and qualitative datum. The key views of this dissertation presented in the front chapters are scrutinized and a creative explanation of CMS institution change are drawn in this chapter.
