Objective:We observed the clinic effect of electroacupuncture treatmenton DPN through clinical trial and discuss the possible mechanism of effectselectroacupuncture treatment on DPN through experimental research.Method:52 DPN patients conforming to the diagnostic criteria wererandomly divided into the electroacupuncture group(26patients)and controlgroup(26patients).Based on the basic treatment,the electroacupuncturegroup was given electroacupuncture.The control group was given intramu-scular injection the methycobal(mecobalamin),treatment the patients for 4weeks.The changes in the symptoms,Toronto clinical scoring system(TCSS),fasting blood glucose(FBG)and nerve conduction velocityobserved in the patients.And observe the effect of electroacupuncture onblood sugar,sciatic nerve conduction velocity,MDA,SOD,T-AOC in bloodserum and sciatic nerve,AGEs,NF-κBp65 and RAGEmRNA level in sciaticnerve on model rats induced by streptozotocin.Results:The clinical study showed that the electroacupuncture treatmenthad good therapeutic efficacy in DPN,could effectively improne the clinicalsymptoms,the total effective rate was 88.46%,could decrease the TCSS andimprove the nerve conductive velocity,There is significant differencebetween the electroacupuncture group and the control group.(P<0.01or0.05).Animal experimental research also showed that electroacupuncturetreatment can improve the nerve conductive velocity,reduce the changes ofpathological morphologyand ultrastructure in sciatic nerve,could attenuatethe content of MDA in serum and sciatic nerve,elevate the content of SOD,T-AOC and decrease the expression of AGEs and NF-κBp65,mRNA ofRAGE in sciatic nerve.Compared with methycobal-control group andacupuncture-treated group,the difference were significant(P<0.05)Conclusion:1,This research subject suggest that electroacupuncture treatment is an effective method for DPN,it can improve clinical symptoms of DPN patients,decrease the TCSS and improve the nerve conductive velocity。2,Electroacupuncture treatment can lessen the damages to structure ofsciatic nerve and protect the function of sciatic nerve in diabetic rats.3,Electroacupuncture treatment have the effect of anti-oxidative stressduring decreasing the level of MDA and increasing level of SOD and T-AOCin serum and sciatic nerve.4,Electroacupuncture treatment can inhibit the expression of AGEs,NF-κBp65 and RAGE mRNA in sciatic nerve,inhibit the biological effects ofAGEs combined with RAGE。thus it can protect nerve cell from oxidativeinjury and reduce oxidative damage to never and vascular,furthermore it canimprove structure and function of peripheral nerves in diabetic rats,This maybe one of the function mechanisms of treatment effects of electroacupuncturetreatment on diabetic peripheral neuropathy.
