The Studies On TCM Symdrome Of ALI And The Experiment Reaction Of Ford Nervilia Leaf On ALI

1 ObjectiveALI and ARDS are the main reasons to cause the death of intensive care. At present, SIRS is well known that it correlates closely with ALI/ARDS. And also causes MODS. The clinical symptoms of ALI often happen at the very beginning during the development of MODS. It brings out the main symptoms. Later on, MODS may develop into MOF and cause death. MOF behaves in the lungs which so called ARDS. SIRS can be caused by two reasons—infectious and non-infectious. The uncontrolled inflamed response is induced by an activated inflamed redical. It expands in Pyaemia and MODS. Up to now, the reasons and pathogenesis are still unknown. And there are no effective therapeutic methods. Detecting, diagnosis and treatment play a big role for SIRS. It can reduce the induction of ALI/ARDS and accelerate recovery.Although many systematic researches have showed that a variety of treatment in traditional Chinese medicine may be valuable for ALI/ARDS, many of them are just the experience from clinical symptoms and at the primary stage. There is a lack of systematic research and relatively accordant understanding in the aetiology. And fewer of them mention about Chinese medicine treatment on ALI/ARDS. To learn more about them, the studies and the therapy will observe the data of the research. And the studies will base on TCM theory. Besides, the research of ALI/ARDS which base on the theory of Chinese medicine therapy. To observe the interruption of endotoxin on ALI /ARDS, the research contrasts between the Herba seu Rhizome Nervilia Fordii(clear heat and tixicant) parenteral solution and ShenMai parenteral solution(subsidy). 2 Methods and subject matter(1)According the standard diagnosis of ALI/ARDS which drafted by SCCM and ACCP in 1992s. Base on the studies of 150 ALI/ARDS cases who were hospitalized in the first affiliated hospital of Guang Zhou TCM University from June 2003 to June 2007. They collected the information from them. Traditional Chinese medical features of different symptom types were investigated. The symptom were found out, also the irregularity.(2)The experiment use LPS to induce ALI/ARDS. It separates into two parts: a. to test on the mouse. b. to cultivate bacteria in external cell environment. Use ELISA to observe the effect of Herba seu Rhizoma Nervilia Fordii parenteral solution and ShenMai parenteral solution serum which act on the IL-6, IL—10, ICAM-1, MCP-1, TNF-α.3 Results3.1 TCM syndromes results:(1)In the ALI/ARDS case, the most common TCM syndromes are phlegm heat syndrome, phlegm moist syndrome, and stagnant blood syndrome. The cases will sometimes with deficiency syndrome.(2) In the ALI/ARDS cases, the percentage of excess syndrome is 56%, which of the excess and deficiency syndrome is 43.3 %. Usually, the excess and deficiency syndrome is more series than excess syndrome. Its death rate is higher than excess syndrome. It points out the illness rate increase and become more series, when excess and deficiency syndrome appears on ALI/ARDS intensive care.3.2 Internal experiment results:(1) The serum of Herba seu Rhizoma Nervilia Fordii parenteral solution and ShenMai parenteral solution groups both can suppress IL-6. During the experiment, it shows the serum of Herba seu Rhizome Nervilia Fordii group gets the better results. (P>0.05)(2) The serum of Herba seu Rhizoma Nervilia Fordii parenteral solution group can suppress the secretion of TNF-α.(3) The serum of Herba seu Rhizoma Nervilia Fordii group gets the better results than ShenMai group on suppress the inflamed cell factor.3.3 External experiment results:(1) The serum of Herba seu Rhizoma Nervilia Fordii parenteral solution and ShenMai parenteral solution groups both can suppress the IL-6. During the experiment, it shows the serum of Herba seu Rhizoma Nervilia Fordii group gets the better results. (P<0.05)(2) The result shows obviously that the serum of Herba seu Rhizoma Nervilia Fordii parenteral solution group can suppress the secretion of TNF-α. (P <0.01)(3) The serum of Herba seu Rhizome Nervilia Fordii group obviously stimulates the secretion of IL—10.(4) Both the serum of Herba seu Rhizoma Nervilia Fordii parenteral solution and ShenMai parenteral solution groups can suppress the secretion of MCP-1. And the serum of Herba seu Rhizoma Nervilia Fordii parenteral solution group gets the better results.(5) The serum of Herba seu Rhizoma Nervilia Fordii parenteral solution group shows a better result than ShenMai group on improve the balance between anti-inflamed factors and inflamed factors.4 Conclusions(1) Heat, phlegm and stagnation are the main pathogens of ALI/ARDS. Deficiency mentions the illness becomes worst. The studies lay out that recognized the levels of syndrome by using TCM methods can help to analysed the condition and arbitrate the recovery of illness.(2) Herba seu Rhizoma Nervilia Fordii can suppress the secretion of TNF-α, IL-6, and MCP-1. It also stimulates the secretion of IL—10. According to the results, it means Herba seu Rhizoma Nervilia Fordii (clear heat and toxicant)can improve the balance between anti-inflamed factors and inflamed factors. And it is more efficiency than ShenMai parenteral solution. It can be use to prevent ALI/ARDS.
