ObjectiveTo observe the effects of cluster needling of scalp point (CNSP) combined with task-oriented training on forelimb motor function and expression of growth associated protein(GAP-43), synaptophysin (SYN), glial fibrillary acidic protein (GFAP), vimentin(Vim) and S100β in rats after focal cerebral infarction. To discuss the probable mechanism that the method promotes the neurological function recovery in rats after focal cerebral ischemia.Methods1.To divide96rats into four groups randomly:sham operation group, model group, task-oriented training group and treadmill running training group. And then each group were divided into four subgroups in accordance with the3rd,7th,14th,21st days(n=6).Endothelin-1(ET-1) was used to induce focal cerebral ischemia model in rats with forelimb dysfunction. The rats in sham operation group and model group were without any intervention, the rats in task-oriented training group were trained by forelimbs reaching training, the rats in treadmill running group were given treadmill running, forelimb motor function were evaluated by net screen test and Immunohistochemistry staining was used to observed the expression of GAP-43,SYN at each time point of the rats in each group a.2.To divide120rats into five groups randomly:sham operation group, model group, task-oriented training group, CNSP group and CNSP combined with task-oriented training group. Forelimb motor function were evaluated by forelimb grasping test and Immunohistochemistry staining was used to observed the expression of GAP-43,SYN GFAP, Vim and S100β in cortical ischemia area at each time point of the rats in each group after modeling.Results1.Scores of net screen test:To compared with the sham operation group at each time point, the net screen test scores of model group improved (P<0.05); To compared with the model group, the scores of net screen test in the task-oriented training group had significant differencefrom the14th day after the modeling (P<0.05); and it in the treadmill running group enhanced from the21st day after the modeling (P<0.05); The scores in the task-oriented training group reduced compared with the treadmill running group, its difference was significant (P<0.05) on the14th and21st day after the modeling.2. Success rate of forelimb grasping:To compared with the sham operation group at each time point, forelimb motor function reduced in each group (P<0.01). To compared with the model group from the3rdday after the modeling, the rate in CNSP combined with task-oriented promoted (P<0.05); on the14th and21st day after the modeling, to compared with task-oriented training group and CNSP group, the rate in CNSP with combined with task-oriented group were improved clearly (P<0.05).3. the expression of SYN around ischemic cortex:To compared with the sham operation group, on the3rd day after the modeling, synaptic pigment optical density value in the model group decreased (P<0.05); on the14th and21st day after the modeling, to compared with treadmill running group, it in task-oriented training group were promoted clearly (P<0.05); to compared with the task-oriented training group and the CNSP group, the expression of SYN in the CNSP combined with task-oriented group improved (P<0.05).4. The expression of GAP-43around ischemic cortex:To compared with the sham operation group on the3rd day after the modeling, GAP-43positive cells in the model group improved (P<0.05); on the14and21st day after the modeling, there were statistically difference between task-oriented training group and treadmill running group(P<0.05); and the expression of GAP-43in the CNSP combined with task-oriented training group is better than it in the CNSP group and task-oriented training group(p<0.05).5. The expression of GFAP around ischemic cortex:On the7th and14th days after the modeling, to compared with the model group, the expression of GFAP in the CNSP combined with task-oriented training group, the task-oriented training group and the CNSP group enhanced (P<0.05), and they in the CNSP combined with task-oriented training group were superior to the task-oriented training group and the CNSP group (P<0.05) respective. There were statistically difference amongst in the CNSP combined with task-oriented training group, the task-oriented training group and the CNSP group (P<0.05). On the21st day after the modeling, the expression of GFAP in the CNSP combined with task-oriented training group were superior to the others (P<0.05).6. The expression of S100β around ischemic cortex:on the3rd and7th day after the modeling, there were statistically difference between the sham operation group and other groups (P<0.05). On the3rd day after the modeling, to compared with the model group, S100β expression promoted in the CNSP combined with task-oriented training group and in the task-oriented training group (P<0.05), on the14th day after the modeling, there were statistically difference between the CNSP combined with task-oriented training group and model group (P<0.01). On the21st day after the modeling, there were statistically difference amongst in the CNSP combined with task-oriented training group, the task-oriented training group, the CNSP group and the model group(P<0.05).7. the expression of Vim around ischemic cortex:on the3rd7th and14th day after the modeling, there were statistically difference between the model group and the sham operation group (P<0.05); on the3r and7th day after the modeling, to compared with the model group, Vim positive cells expression in the CNSP combined with task-oriented training group, in the task-oriented training group and in the CNSP group improved (P<0.05); From7after the modeling, Vim positive cells expression declined in the CNSP combined with task-oriented training group, but there were statistically difference between the CNSP combined with task-oriented training group and others (P<0.05).Conclusions1.The task-oriented training can enhance the forelimb motor function after focal cerebral ischemia in the rats, its effect is superior to treadmill training.2.The task-oriented training can promote the expression of GAP-43and SYN around the ischemic area of focal cerebral ischemia in rat cortex, its effect is better than treadmill training.3.The method of CNSP with combined with task-oriented training can improve the forelimb motor function after focal cerebral ischemia in the rats, its effect is superior to the task-oriented training or CNSP respective.4. The method of CNSP with combined with task-oriented training can promote the expression of the GAP-43and SYN around the ischemic cortex of rats after focal cerebral ischemia, Thereby contributing to the nerves sprout and new synapse formation, its effect is superior to the task-oriented training or CNSP respective.5. The method of CNSP with combined with task-oriented training can promote the expression of GFAP,S100β and VIM around the ischemic cortex of rats after focal cerebral ischemia, and then they can facilitate glial activating so as to reduce the damage neurons, its effect is superior to the task-oriented training or CNSP respective.
