Hepatitis and hepatic cirrhosis are common hepatic diseases in China,the hepatic cells are generally inolved that induce the liver functional disorder and liver function failure. The decrease of hiso- regeneration capacity acts as the importent character of advanced hepatopathy and key factor of clinical symptoms. Accompany with the development of hiso-project research, treating advanced hepat- opathy by self adipo- desmohemoblast stem cells transplantation shows the most hopeful developing prospect. Adipose- desmohem- oblast stem cells(ADSCs) has became another focus in the domain of stemal cells research and is regarded as an excellent self-seed-cell in hiso-project.So we selected ADSCs as the research object and made initial exploration in related experimential and clinical application directions.1 Seperating, culture and identification of adult ADSCsIn this research, we used the gradient centrifugation in vitro to separate and purify the ADSCs .After the serial subcultivation for three generations,the ADSCs were identified by detecting the CD44 monoclonal antibody on cell surface; Then the HGF(20ug/ml)+ FGF -4 (10ng/ml) were used as the differentiation induceing factor to treat the ADSCs.The induced ADSCs were identified by detecting the AFP monoclonal antibody in endochylema. The result of research proves that enough adult adipo- desmohemoblast stem cells can be extracted from adipose tissue and propagate stablly in vitro.The adipo- desmo- hemoblast stem cells have the potentiality that can differentiate to certain cells whose function are similar to hepatic cells.2 Migration and distribution of ADSCs transplanted through hepatic arteryThe rat liver cirrhosis models ,which were made by intraperitoneal injection with carbon tetrachloride,were diveded randomly into group of transplanting ADSCs through hepatic artery, group of transplanting ADSCs through portal vein and control group(10 rats per group). 1.0 ml suspension of ADSCS marked by CFSE were injected into hepatic artery(group of transplanting ADSCs through hepatic artery)and portal vein (group of transplanting ADSCs through portal vein),while 1.0ml normal saline were injected into hepatic artery in control group.All the rats were killed on the 22th days after the transplantation,the livers were taken out and made into slices by quick freezing.The slices were directly observed and taken photos under fluorescent microscope,the distribution of fluorescent cells were observed and the number of fluorescent cells in 10 visual fields. The result of research proves that the transplantation of self ADSCs though angio-path can gain ideal distributation in liver.The transplantation through hepatic artery and portal vein can lead to the similar result,but the hepatic artery path may be more reasonable and feasible according to the blood-supply character of live and the status of clinical application.3 The initial clinical research on advanced hepatopathy treated by self adipo- desmohemoblast stem cells transplantationAll the 18 clinical cases were selected from the in-patients of posth- epatitic cirrhosis accompany with portal hypertension whose hepatic functional classification were B class.8 patients were divided into treating group treated by self adipo- desmohemoblast stem cells transplantation and the rest 10 patients were divided into control group treated by synthetic medical treatment. 200ml adipose tissue were extracted from every patient of treating group.After the abstraction, culture and hepatocyte-growth inducing procedure ,the ADSCs were transplanted into the liver through hepatic artery. On the 1th,2th,4th,8 th week post-transplantation, the hepatic functional datas of each patient including ALT,TBIL,ALB were detected. At the same time ,we observed the adverse reaction and the improvement of clinical symptoms such as ascites,appetite,physical strength, abdominal distension,sleep etc while comparing the same aspects of patients in control group. According to the comparison of above hepatic functional datas and clinical symptoms ,we can draw the following conclusions : Self adip o-desmohemoblast stem cells transplantation can obviously improve the hepatic function of advanced hepatopathy patients and can be regarded as a effective cyto-substitutive therapy besides orthotopic liver transplantation; The interventional self adipo-desmohemoblast stem cells transplanta- tion through hepatic artery not only can implement the liver-target transplantation of ADSCs, but also posses broad application prospect in celluar transplantation domain.
