Effects Of Photon Intervention On Rat In Early Stage Of Endotoxemia

Objective: Investigated the changes of circulating hemocytes, arterial blood gas, early inflammatory media (TNF-α, IL-1β, IL-6, IL-8), mesenteric microcirculation and gastric mucosa, evaluated the possible effects of photon intervention on rat in the early stage of endotoxemia (ETM).Methods: 60 healthy male rats were randomly divided into three groups: control group, endotoxin injury group and photon intervention group, each group included 20 rats which were subdivided into four temporal subgroups (1h, 2h, 4h, 6h respectively). The disease model of ETM was established by injection of lipopolysaccharide (LPS) at the dose of 5mg/kg into thigh vein. In photon intervention group, the rats were irradiated by SLT semiconductor laser (wavelength 650nm, 5mW) on the femoral artery and vein, otic blood vessel concurrently for 30min. The interference course was vertical irradiation taken at 30min after the injection of LPS. The changes of circulating hemocytes and arterial gas analysis were studied. Radioimmunoassay method was adopted to compare the levels of TNF-α, IL-1β, IL-6, IL-8 before and after photon intervention. The mesenteric microcirculation was investigated in ETM-affected rats. Then the pathomorphological method was used to value the probably protective effect of photon intervention on gastric mucosa in the early stage of ETM.Results: (1) In endotoxin injury group, the number of blood white cell decreased and got its bottom at 2h-4h while the circulating platelet reduced obviously at 6h in comparing with the control group. pH of arterial blood dropped only at 1h while the value of lactic acid had no clearly increased. PaO2 decreased rapidly and progressively but PaCO2 was not found increased. In the blood circulation, normal rats had a definite amount of TNF-α, IL-1β, IL-6, IL-8 active protein. After ETMoccurred, the levels of those inflammation mediators in circulation rose significantly. After photon intervention, compared with endotoxin injury group at the same stage, the number of blood white cell rose at 6h, PaO2 significantly increased. TNF-α was subjected to significant inhibition within the initial 6h, while IL-1β and IL-8 were subjected to significant inhibit distinctly. (2) As for microcirculation, after ETM occurred, the blood flow velocity in microvein accelerated at 1h-2h, and decelerated at 6h. More leucocytes stuck to vessel wall and abnormal blood flow state was seen, while blood flow volume of mesenteric vascular still stayed at the comparatively normal level in the initial stage. When intervention by photon, compared with endotoxin injury group at the same stage, the blood flow velocity was smooth and stable, leucocytes stuck to vessel wall was difficult to find and the microcirculation flow state got better clearly. (3) The pH of gastric juice had no change within 6h after ETM took place, but from the pathomorphological observation it was shown that the gastric mucosa tissue of rat might express an inflammatory change, mainly in the form of epithelial cell impaired, exuded and infiltrated of inflammatory cell, which was found at 6h after the injury. After photon intervention, when compared with the ETM model group, the inflammatory reaction of gastric mucosa tissue was abated and epithelial cell impairment, exudation, infiltration of inflammatory cells were all bettered apparently.Conclusions: (1) The changes of metabolism, microcirculation and gastric mucosa were highly related to the ETM incidence. (2) Photon intervention attenuated the changes of blood white cell and platelet in circulation, improved oxygen supply, inhibited the expression of early inflammatory mediators. (3) Photon intervention may ameliorate microcirculation, alleviate the injury of gastric mucosa. (4) Photon intervention demonstrated the potential adjuvant treatment value on ETM in early stage.
