The utilization of homologous tendon grafts in the reconstruction of flexor tendons have been overlooked by the reserch communities for unknow reason during the last two decades. Recently,there is a disconcerting tendency to immediately opt for conventional autologuos tenden grafting or staged flexor tendon reconstruction without consideration of their limitations both in the source and in the final achievement of functional restoration.The available autogenous grafts used clinically are oragenated from extrasynovial source which are structurally and biologically differs from intrasynovial tendons. The results of extrasynovial tendon graft was usually compromised by numerous complications including extensive adhesion formation ,steefness of joint and poor fuction. Recent evidence that mobilized intrasynvial tendons have the capacity for adhesion-free healing suggests that the poorer results obtained with traditional methods of tendon-grafting may be related to the transplantation of extrasynoivial tendons to an intrasynovial enviroment.
