ObjectivePrimary Sjogren's syndrome (pSS) is a slowly progressive inflammatory autoimmune disorder characterized by lymphocytic infiltration of exocrine glands leading to keratoconjunctivitis sicca and xerostomia. The characteristic abnormality found on the lacrimal or salivary gland biopsy is the lymphocytic infiltration in the glandular lobules. The main clinical manifestation are as follows: decreased saliva production leading to difficulties with chewing, swallowing, and even speech,severe dental caries;dry eyes with reduced tear production, gritty or sandy sensation under the lids. In about 2/3 patients, systemic damage out side of exocrine glands could be found, including musculoskeletal symptoms, gastrointestinal symptoms, cutaneous findings, pulmonary findings and renal findings etc. The female -to -male ratio is approximately 9:1. It primarily affects women from the fourth to the fifth decades of life. Epidemiological studies in Chinese show prevalence rates are 0. 29%~0. 77%. Among aged people, the prevalence rates are 3%~4%.Although the pathophysiology of Sjogrens syndrome is not yet fully understood, there is increasing evidence that tumour necrosis factorα(TNF -α) and interleukin (IL - 1β) , are important mediators of the autoimmune response and have a potential role in mediating tissue destruction of salivary and lachrymal glands. TNF - α and IL1 - β have been shown to inhibit both basal and stimulated secretion. They also stimulate the production of collagenases. Both cytokines enhance the secretion of matrix metalloproteinase 2, synthesised by acinar cells, and thus may promote disruption of acinar cell architecture.
