Clinical pathway is an effective way for improving quality of medical care service and controlling its cost, which has been used in many countries. Practice showed that clinical pathway played an important role in improving quality of medical care service, controlling its rising cost, and standardize medical care service. It is a good tool in management of medical care service and decision-making by clinicians, governments and units for medical fee repay. Community health care service can meet the needs of the people for health care and provide them cheaper, convenient and comprehensive medical care service. This model of medical care service has been implemented in most cities in China, and some successful experiences have been obtained, but there still existed some problems, such as fewer patients visiting local community health care service center, etc., which would restrict the development of community health care service. In order to provide quality health care service for local community residents, it is necessary to introduce clinical pathway into medical care service and to establish two-way referral system between community health care service centers and hospitals in China.Objectives1. To develop clinical pathways of four kinds of common chronic non-communicable diseases, i.e., hypertension, coronary heart disease (CHD), type 2 diabetes mellitus (2-DM) and stroke.2. To develop two-way referral system between community health care service centers and hospitals for the patients with these diseases.
