In hand surgery, the skin defect exposed vessel, nerve, tendon, bone or joint required flap reconstruction. The optimal requirement included: Reliable skin cover;Good sensibility;Without shorten the length of the finger; Excellent function and cosmetic results; Simple and one — stage procedure; No secondary deformity at the donor site. To our knowledge, none of the methods could achieve the requirements above — mentioned. With the extensive use of venous flap, the experimental and clinical studies of the dorsal digital venous flap with perserving the perivenous tissue were performed. The aim was to find a surgical method that could reach the optimal requirments for the reconstruction of the skin defect.Twenty adult hands from cadavers were used to study the vesseles in the dorsum of the finger with careful dissection and mensurement. The results :(1)The superiornutrient artery to the skin in the dorsum of the finger was the cutaneous branches of the proper palmar digital artery, the dorsal digital artery was short and small, taking part in the blood supplies of the dorsal digital skin of the proximal phalanges. (2) The veins communicated freely each other, and form the first, the second and the third venous arch in the distal phalangeal joint level, the proxi-...
